Job summary

  • This advert is for an Advanced Frailty Nurse Practitioner to work primarily at St George's Hospital, Tooting.
  • We welcome applications from skilled nurses to compliment our team of expert frailty practitioners from AHP and nursing backgrounds.
  • The post holder will work in a liaison role with a highly skilled multidisciplinary team, reviewing frail older adults at the 'front door' (acute settings), while working closely with the ED and AMU teams
  • The post holder will be practicing autonomously as an ACP to provide patient-centred quality care.
  • This will encompass the skills of advanced assessment, examination, diagnosis and management within an agreed scope of practice.
  • To work within the Multi-professional framework for advanced clinical practice in England (HEE 2017).
  • The post holder will deliver an 80% clinical component to their role and 20% related to leadership and management, education and research.
  • The post holder will work to develop the front door frailty service. This will involve working closely with Senior Health, ED, and AMU MDTs. With support from Consultant Geriatrician colleagues and a Consultant frailty practitioner, you will support the operational and clinical leadership of the Older People's Assessment and Liaison (OPAL) team.

Main duties of the job

  • The post holder will work to develop the front door frailty service.
  • This will involve working closely with Senior Health, ED, and AMU MDTs. With support from Consultant Geriatrician colleagues and a Consultant frailty practitioner, you will support the operational and clinical leadership of the Older People's Assessment and Liaison (OPAL) team.
  • The post holder will be part of a highly skilled multi-disciplinary team and with support from the current OPAL team lead and OPAL consultant frailty practitioner, you will be responsible for the day-to- day operational management of the OPAL Team. You will also provide clinical leadership to the frailty practitioners.
  • The post holder will be responsible for leading the initiation of a Comprehensive Geriatric Assessment (CGA), delivering high quality care and avoidance of unnecessary admission for frail older adults. With the OPAL team, they may also deliver targeted CGA to frail older adults across the medical wards.
  • The post holder will be a leader for service development within the OPAL team, aiming to improve standards for frail older adults in the hospital and beyond into the community.
  • The post holder will also take on an educational role, supervising the frailty practitioners and developing programmes of education and training to improve standards for frail older adults in the hospital and beyond into the community.
