Job summary

  • To provide specialist high quality care for patients with spasticity.
  • The post holder will provide strategic clinical insight into all aspects of the patient pathway and be expected to perform duties with a high degree of clinical skill and knowledge.
  • The post holder will be expected to work closely with the Multi-Disciplinary MDT team and ward staff managing patients with spasticity and ITB pumps.
  • To provide a seamless co-ordinated and timely approach to spasticity.

Main duties of the job

  • To work in an MDT to deliver specialist care to patients who have severe spasticity.
  • To promote independence and engagement in their spasticity management including patients with ITB pumps and following national standards.
  • To provide a specialist nursing advise and support to patients, their families and other health care professionals.
  • To work as an autonomous, advanced practitioner and effectively deliver an expert nursing service within the specialty of spasticity and intrathecal Baclofen.
  • Provide effective hands-on nursing care, support, supervision and education to clients, carers, and relatives with complex neurological needs.

Job responsibilities

Provide clinical expertise underpinned by theoretical specialist knowledge following best practice and evidence based advise to patients carer and healthcare professionals.

To be actively involved in patient care and working autonomously ensuring patients have an accurate plan reflecting assessment and review.

Be able to demonstrate advanced clinical skills and expertise to improve Quality of Life.

To be able to provide effective decisions to address complex issues with the patient at the centre of the decision.

Provide specialist advice to Neurologically impaired patients with spasticity and their families to improve clinical outcomes influencing care and treatment.

Act as a lead on clinical care providing rationales, set standards of care and monitor progress.

Provide reports and recommendations for further treatment/care as part of the discharge planning process for patients admitted with spasticity. To liaise with the Discharge Co-ordinator and ward staff to ensure arrangements are in place for a safe appropriate discharge.

To lead on key nursing quality initiatives within the neuro pathway including bladder and bowel continence retraining/management, prevention of pressure sores, self-medication management, falls and urinary infections and prevention with a particular focus on the specialist needs of patients with spasticity.
