Job summary
Working with the Cardiology Nurse Practitioners (Band 7), Cardiologists, Patient Pathway Co-ordinators and others, the Lead Nurse will have a key role in the ACS pathway. They will work closely with other areas and specialities and be involved in scheduling to ensure that the Catheter Laboratory and ward resources are used to their maximum and meeting government waiting time targets and local delivery plans. They will also oversee the running of the rapid access chest pain (RACPC) nurse led service
In addition, the post holder will be expected to participate in the strategic development of the service by:
- To be a highly visible and proactive leader acting as a clinical resource and ensuring the sustained delivery of high standards of nursing care and service delivery.
- To play a key operational role and maintain a proactive approach to managing patient flow through the service.
- To be responsible for exploring current practice, identifying areas for development, and planning the appropriate change using current evidence to evaluate outcomes.
- Providing leadership, purpose, and vision.
- Establishing and developing relationships with key stakeholders involved in the management of chest pain services both internally and externally.
- Actively disseminating best practice through publication and conference presentations and participating in nursing research and national working parties.
Main duties of the job
In close collaboration with other members of the multidisciplinary team, the Lead Nurse Practitioner carries responsibility for overseeing and delivering an effective patient pathway from referral through to treatment for non-elective patients with ACS.
Practicing as an autonomous clinician within the multi-professional team and using technical and organisational skills, nursing expertise and clinical decision-making skills, the Lead Nurse will:
- Monitor the patient's progress along the patient pathway
- Identify and manage any barriers to their progress to treatment
- Carry out clinical assessment of patients
- Identify, order and review investigations
- Create appropriate treatment plans to prepare patients for Treatment
- Refer to other clinicians (as required)
- Provide education & support
- Act as a point of contact for patients and their next of kin
