Job summary
Following service growth and re-configuration, we are looking to appoint a dynamic colleague to complement the existing multidisciplinary intestinal failure team at St George's. The successful candidate would contribute to the continued development of the service including our collaboration with St Mark's, as well as other regional initiatives. Other complementary interests, for example in IBD or endoscopy, are encouraged.
The successful candidate would deliver inpatient and outpatient care to our cohort of HPN patients as part of a multidisciplinary team including two existing consultant gastroenterologists, three CNS, two dietitians, and a specialist pharmacist, and would work closely with allied specialties including intestinal failure surgeons and interventional radiology.
The post holder will also contribute to the delivery of inpatient and outpatient general luminal gastroenterology services including diagnostic and (where appropriate) therapeutic endoscopy together with 8 other luminal consultant colleagues (with a further 9th post approved). There is no acute medicine commitment, but the post holder will be part of a 1:15 consultant led, registrar delivered GI bleed rota including 1:15 weekend consultant ward rounds of gastroenterology / hepatology inpatients.
Main duties of the job
The successful candidate would be expected to provide high level Gastroenterology care. Sessions will take place St George's Hospital, Tooting but may include sessions at Queen Mary's Hospital, Roehampton or the Nelson Health Centre, South Wimbledon.
The post holder will be responsible for the care of Gastroenterology inpatients in a 1:9 Consultant rota. The post holder will be expected to have expertise in the management of complex nutritional problems and intestinal failure, supported by a dynamic multidisciplinary team.
The successful candidate will deliver a total of 2.5 outpatient clinics per week, including a specialist IBD clinic and a specialist complex nutrition / IF clinic on alternate weeks, and a suspected upper GI cancer clinic.
St George's Hospital is a national endoscopy training centre, in a JAG-accredited unit, and the SW London Bowel Cancer Screening Centre. The post holder will provide diagnostic and therapeutic upper and lower GI endoscopy, contributing to the training of trainee Gastroenterologists, Surgeons and Nurse Medical Endoscopists. There will be opportunities to participate in the delivery of national JAG training courses at the St George's site.
Other subspecialty interests would be considered (e.g. IBD, neurogastroenterology, advanced endoscopy), in which case additional responsibilities may accrue.

The Intestinal Failure (IF) Service at SGH provides specialist assessment, review and monitoring of Type I, Type II and Type III IF patients in both in-patient and out-patient settings, including discharging patient on HPN. These patients are managed by a multi-disciplinary nutritional support team (NST), currently comprising two Consultant Gastroenterologists, three nutrition nurse specialists, a specialist pharmacist, and two senior dietitians.
There are close working relationships with key related specialties including IF surgery (colorectal and bariatric / uuper GI) and interventional radiology. Where possible, all IF in-patients are cohorted to Gray (surgical) or Allingham (medical) wards to receive the best possible care by nurses who have received specific training in the provision of PN. The nutrition service also receives referrals for complex enteral feeding challenges and is the Lead centre for Medical Emergencies in Eating Disorders (MEED), with close links to the local Severe Eating Disorder Unit (SEDU) at Springfield Hospital.
The HPN service currently has approximately 60 patients, a significant number of which are referred from external Trusts in the South West Thames region for medical and surgical management of intestinal failure. There are existing strong links with other regional hospitals including the Royal Marsden Hospital through the South West Thames IF Network (SWIFTNET). In 2021, St. Marks (as lead provider) and St. Georges were jointly commissioned by NHSE to provide a regional medical and surgical service for patients with Severe Intestinal Failure (SIF) in West London. The two Trusts work closely together and operate a single hub for referrals, with patients from SW London generally triaged to St. Georges and NW London to St. Marks. Further exciting opportunities being explored include establishing outreach services and closer collaborative working with our surrounding Trusts including the Royal Marsden Hospital. The post holder would be expected to be involved in developing these collaborations further.
