Job summary
The Patient Pathway Coordinator is responsible for the overall management of pathways for all elective who require admission under the care of the Urology department.
- To ensure the smooth running of elective and day case admissions.
- To schedule theatre lists in liaison with patients, consultants and capacity.
- To ensure all appropriate pre-operative assessments have been carried out to allow theatre lists to run as planned.
- To manage/ validate a large waiting list and to assist in ensuring all patients are operated on within the maximum waiting time targets.
- Ensure that patients are informed with reasonable notice, and that all relevant paperwork is completed for given admission dates. This includes ensuring that all elective attendances/admissions are promptly and accurately recorded on the appropriate system(s)
- To ensure accurate and complete computer records are maintained for patients.
- To contribute to service developments.
- To train new admissions staff to ensure standards of service across specialties is of a uniformly high standard.
- To ensure all patients are operated on within the maximum waiting time targets.
- To cross cover the Urology Patient Pathway Coordinators to ensure the smooth running of elective and day case admissions when other members of the team are on leave.
Main duties of the job
The Urology department is looking for a highly motivated, professional and competent patient pathway co-ordinator to work within the Surgery Directorate at St Georges Hospital.The post will be supporting a team of eighteen Urology consultants, which will involve booking and manging patients on routine elective waiting lists for the department. You will also be involved in validation and ensuring waiting lists have no data quality errors of inpatient pathways, ensuring pathway management is in line with the Trust's Access Policy. Patients need to be robustly tracked through the appropriate pathway and ensure that all the relevant data is accurately recorded, reported and analysed in a timely manner in order to meet national and internal targets.We are looking for a candidate with good attention to detail and committed to excellent patient service. The successful candidate will also need to possess excellent Information Technology skills, with the ability to use several computer packages. The post requires the individual to have had experience in an NHS administration setting, with good communication, teamwork and prioritisation skills as the post holder will be dealing with a wide ranging audience and sensitive issues. Experience of using Cerner/CRS Millennium is a desirable attribute, but not essential.
