Macmillan Support Worker-Breast/Macmillan Information Centre

Exciting opportunities have arisen for passionate individuals to play pioneering roles at St. George's University Hospitals NHS Foundation Trust.

We are looking for Macmillan Support Workers to help cancer patients navigate the complex health and social care system during and following completion of cancer treatment. The post holder is an important member of the team, providing information and support to patients and their carers diagnosed with cancer as well as contributing to the holistic needs assessment and the development of an individual care plan under the direction of the cancer specific Clinical Nurse Specialist and the Macmillan Information Centre Team. The aim of the post is to contribute to the improvement of patient care and experience by improving access to the service and seamless coordination of care for people with non-complex care needs.

Candidates who have proven experience as a Health Care Assistant involved in direct patient care within a healthcare environment or equivalent healthcare related experience.

Main duties of the job

The main purpose of the Macmillan Support Workers role is to support people to navigate the complex health and social care system during and following completion of cancer treatment. The post holder will coordinate care by providing a single point of access, including rapid re-entry into the system for those people identified as having urgent or specialist needs; and coordinate care, education and support for people with noncomplex care needs.

Cancer accounts for 10% of all activity at St George's, touching most specialties across the Trust and accounting for almost one in six operations carried out here.

We are a large provider of cancer care diagnosing over 3900 patients with Cancer in 2021 going on to offer around 5500 treatments. These treatments include Systemic Anti-Cancer Treatments, Surgery, best supportive care, and palliative care.

The post holder will provide support to patient with all types of cancer within the Macmillan Cancer Centre. They will also have a specific role within the Breast Cancer Team supporting a group of patients with breast cancer. They will be supported by an experienced team of Macmillan Support Workers and the Lead Nurse for Breast Cancer and the Macmillan Information Centre Manager. The post holder will be required to work across multiple hospital sites
