Job summary

Croydon Falls Service aims to prevent and reduce the impact of falls and fracture on older people in Croydon. The post-holder provides a clinical lead in providing falls and bone health assessment to adults in their relevant community setting, and working closely with primary care and other community sectors in identifying patients with risk of falls and future fractures and to provide appropriate management plan.

This post will work closely with other members of the falls team, including Consultant geriatricians, Physiotherapists and Occupational therapists, Nurses and Falls Service Co-ordinator.

Main duties of the job

The post holder will be required to develop and run the Fracture Liaison Service (FLS) with the other members of the Falls and Bone Health Service (FBHS), which includes:

  • Case-finding, across all sectors, assessing, performing diagnostic evaluations.
  • Making recommendations for secondary prevention of osteoporotic fractures.
  • Working with the other member of the falls services to ensure falls assessments are performed.
  • Providing input into the Falls Service as appropriate in line with agreed guidelines.

The post holder is an autonomous practitioner and clinical lead, providing specialist falls and bone health assessment, intervention and advice for Older People in Croydon who have fallen or are at risk of falling.

  • To communicate complex and sensitive information in an understandable format to patients identified by the Fracture Liaison Service.

  • To communicate effectively with patients and /or carers, appropriate complex information regarding the patients medical condition and inform them of the benefits and risks of suggested treatment options.

  • To communicate complex information to a multicultural and ethnically diverse client group, where there are potential barriers to understanding e.g. patients with senile dementia, cognitive deficits or denial.

  • To maintain accurate and up to date records on patients treatment in line with the guidelines of the professional body and ensure that more junior and unqualified staff do likewise.

  • To co-operate with other senior staff in standardising record keeping in accordance with the Trust policy

  • To set and maintain a high personal standard of work, and encourage junior staff, TIs, assistants and students to do the same, in order to teach them and monitor their standards, ensuring an ethical sensitive and culturally aware approach.

  • To keep professionally up to date in clinical practice, especially relating to patient care within the field of Falls and Bone health and maintain a CPD portfolio.

  • To be pro-active as a consultant to other staff in the stated field and be willing to contribute to multi-disciplinary training programmes.
