In Croydon, Health and Social care providers and commissioners have formed the Croydon Alliance Board with the aim of providing a whole system transformation that will deliver the outcomes our over 65's have specified that they want.

We require a Healthcare Assistant to work within the Community Nursing Service to provide efficient and effective nursing care to patients in their own homes. The ideal candidate should have experience working within a Health Care setting, be compassionate and able to communicate effectively with patients.

All candidates must have a clean driving license and use of a car for work purposes. Once recruited staff can take advantage of our child care facilities.

Main duties of the job

MAIN DUTIES AND RESPONSIBILITIES

1. To visit patients in their own home to provide care and interventions as delegated by a registerednurse in accordance with an individualised care plan.

2. To provide feedback to the registered nurse, colleagues and other professionals regardingthe patient's health, wellbeing and changing needs.

3. To manage and prioritise delegated patients visits.

4. To be an effective member of the nursing team and work as part of the wider Primary Health CareTeam

5. To actively participate in Service Development
