Job summary

Virtual Ward Support Worker Band 4 Our vision is for the people of Croydon to experience well-co-ordinated care and support in the most appropriate setting, which is truly person-centred and helps them to maintain their independence.

We are developing the services that we currently provide to reduce hospital admissions and to facilitate earlier discharge from hospital through the use of technology, ensuring patients have the safest best care that is delivered in their home environment via the Croydon Virtual Ward.

TheVirtual Wardservice available 24 hours a day, 7 days a week.

The post holder will require advanced assessment skills and have the ability to monitor and support patients via the Virtual Ward with complex needs through an acute episode in the community in order to avoid a hospital admission. The candidates will work closely with the local community teams and ensure early escalation of unwell patients. They will need to be motivated, innovative with strong leadership and communication skills, to support the development of the Virtual Ward.

Main duties of the job

The post holder will provide a high standard, efficient, cost-effective Virtual Ward service to patients with a range of acute and chronic conditions hosted by Croydon Health Services NHS Trust in partnership with acute and community health service providers in Kingston and Richmond, Merton and Wandsworth, and Sutton. The South West London Remote Monitoring Team (virtual ward) will provide 40-50 virtual beds per 100,000 population across South West London. Patients with a range of acute and chronic conditions will be remotely monitored. This is to support patient discharges from hospital, prevent avoidable in-patient admissions and reduce pressure on inpatient bed capacity whilst maintaining standards of care that areequivalent to those in hospital. The post holder will act as a support for the Virtual Ward team members in monitoring patients who areunder the care of the Virtual Ward. The post holder will be line managed by, and will work closely with, the SWL Virtual Ward team to ensure that there is an effective clinical service for the Virtual Ward Service.The post holder will be working at Croydon Health Services NHS Trust site.

To be able to interpret and communicate to senior clinicians monitored patients' vital signs and symptoms. To be able to monitor patient's progress on the Virtual Ward by understanding normal vital signs. Knowledge of when to seek advice and escalate to an appropriate registered health or care professional.

To be able to implement the care as advised by the senior clinical staff, planning their work for the day based on this and patient need.To be able to record observations, liaising with patients and where required patients families/carers, in providing a positive experience for the patient.

To support senior clinical colleagues in monitoring patients assigned to their care in virtual wards. To support clinical Band 7, ensuring safe practice and all elements of pathway run smoothly. To deliver and evaluate clinical practice which reflects all relevant nationally recognised clinical guidelines.

Associate Nurse registered with the NMC/Awaiting GMC registration

Essential

  • Registered Nursing Associate on the NMC register: sufficient experience or Overseas qualified Medical Doctor awaiting/working towards GMC registration.
  • Educated to a minimum level 5 Foundation degree level specific to nursing associate practice

Desirable

  • GCSEs Grade A-C in English and another subject or equivalent
