Acute Liaison and Frailty Assessment Service (ALFA) Practitioner Band 6 - full time 37.5 hours per week

The Acute Liaison and Frailty Assessment Service is a new team based in the Emergency Department at Croydon University Hospital. The team is multi-professional utilising skills across nursing and allied health professionals and is integral in ensuringa seamless response to those presenting with either frailty related illness or other medical, functional or social reasons to facilitate the discharge of adult ED attenders (18+) who do not require admission.

The Band 6practitioner willwork alongsidetheBand 7 practitioner and B8a ACPs already established within the team.The role is non-profession specific and we welcome applications from both nursing and allied health professionals.If you are a registered nurse or AHP who has significant Band 5/6 experiencein the areas ofassessingfrailty, autonomous practiceand planning supported discharge, this post may be for you.

Main duties of the job

  • The ALFA (Acute Liaison and Frailty Assessment) team is based in the emergency department (ED) at Croydon University Hospital.
  • The team has been formed by merging the existing ED Liaison team with the recently commissioned "front door frailty service" to create a seamless response to those presenting with either frailty related illness or other medical, functional or social reasons to facilitate discharge of adult ED attenders (18+) who do not require admission.
  • The team is a multi-professional team utilising skills across nursing and AHPs. The role is non-profession specific and each practitioner will be required to maintain their own professional registration
  • The team will be integral in ensuring patients receive integrated care across acute frailty pathways, relevant departments and services, and the community enabling teams to work more collaboratively, resulting in patient care being less fragmented or delayed as well as being a more positive experience with better outcomes
