Job summary

JOB SUMMARY

The purpose of the Specialist Health Visitor complex needs role is to be responsible for leading and developing an enhanced service for families with complex needs.

The Specialist Health Visitor will ensure, jointly with other services, the creation of multidisciplinary pathways, policies and procedures to address the complex needs of children and families including those with complex safeguarding issues.

The Specialist Health Visitor will provide relevant consultation, training and support to the universal children's services. The emphasis will be on early intervention, multi disciplinary relationships and the promotion of positive parenting between parents and their infants.

Provide leadership and embed up to date research and evidence based practice and ensure effective dissemination to all Public health nursing staff to ensure the efficient delivery of the Healthy Child Programme. This will include provision/oversight of any required training and Clinical Audit to provide assurance of a competent confident workforce.

Main duties of the job

KEY RESPONSIBILITIES

The Specialist Health Visitor will:

  • Provide a link between health visitors, midwives, GP's, Social care and Early Help services.

Services and other agencies in relation to families where there

  • are challenges to the parent infant relationship and where there are significant safeguarding issues
  • Liaise with other service provision for these families and promote integration of services.
  • Develop comprehensive care pathways for women and families in active collaboration with colleagues. s
  • Participate in any professional and family meetings where appropriate

To be responsible for quality assurance, training and development for Croydon Public health Nursing, embedding evidence based practice.

To act as a resource for the service according to the HV service model as a specialist, providing expert knowledge.

To work collaboratively with key stakeholders to develop joint pathways.
