Inpatient Diabetes Specialist Nurse
Band 7
Part time hours (18.75 hours)
To provide and manage expert specialist clinical practice in Croydon Health Service NHS Trust for patients with diabetes in the borough of Croydon.
To be a source of expertise, knowledge and support, upskilling healthcare professionalson the wards, providing basic education to patients and their significant others.
To develop evidence based clinical nursing aligned with current research and guidelines. To ensure patients and their relatives/ carers are well informed and supported in the management of their diabetes care.
To work with the wider multidisciplinary team in a proactive way with the aim to prevent short and long term complications of diabetes, supporting early discharge and reduce emergency attendance.
To work with the community diabetes service to provide structured education for people with diabetes and their family
To provide hospital nurse led clinicsto support the management of diabetes. This includes initiating diabetes oral medication, insulin and GLP-1 therapy management, and monitoring patients ongoing glycaemic control.

This is an exciting opportunity to work as a member of a specialist multidisciplinary team comprising consultants, nurses, dietitians, psychologist and support staff to develop an innovative integrated service.

We are seeking to recruit diabetes specialist nurses who have a passion for improving the care of people with diabetes, excellent communication skills and enjoy working as part of a team. The role will include working closely with colleagues in hospital and primary care to ensure that the people of Croydon receive high quality, joined up care and avoid unnecessary complications.
A clean driving license and use of a car for work purposes is desirable but not essential.
We offer education/support to help with personal development, Apprenticeship jobs and other new development opportunities.
You will have the opportunity to be part of the specialist services which includes:
- Antenatal care - Diabetes in pregnancy clinics
- Renal clinics
- Endocrine Day cases
- Transition and young person's clinic
- Insulin pump clinics
- Continuous Glucose Monitoring clinics and other Diabetes technologies
- DSN Joint/clinics
