Job summary
An exciting opportunity has arisen for a Patient Pathway Co-Ordinator to join the Pod 3 Administrative Team , Gastroenterology Department.
The post holder will work as part of a multi-disciplinary Point of Delivery team (POD), and will provide support in the proactive management of the patient pathway from referral to discharge, ensuring the entire pathway is managed in a timely manner. The post holder will also act as the point of contact for patients regarding all administrative issues relating to the patients' pathway of care.
Main duties of the job
The post holder will be responsible for delivering an effective and efficient support service to specialty Point of Delivery teams (POD), to assist in the provision of quality patient care. This will include general clerical and administrative duties that support the administrative elements of the entire patient pathway and elective DM01 standard.
This role will be based within our Gastroenterology POD.
