This post is to provide middle grade registrar support to the Acute Care of the Elderly (ACE) Unit and Clinic as part of the acute frailty service at Croydon University Hospital.
The ACE service was started in 2012, which was part of a whole system transformation in the management of the local ageing population. Our aims were to enable specialist care in the right place at the right time for the older frail person in/approaching crisis, improve co-ordination and collaboration between older people's services, and shift patient care closer to home and helping to avoid unnecessary hospital admissions. We also established stronger links between hospital geriatricians and primary care and community services in order to be more responsive to the older person in crisis by starting a hotline and ACE hot clinic.
Main duties of the job
The ACE Unit is a 14-20 bedded short-stay inpatient unit set up to target emergency management of frail older people, offering early Consultant led Comprehensive Geriatric Assessment and multidisciplinary assessments whilst linking with Community, Social Services, Mental Health and voluntary teams.
The post holder will assess patients aged 65 years and above with frailty and geriatric needs and carry out the medical component of the comprehensive geriatric assessment. ACE patients are referred from the Emergency Department and GPs and the MDT includes a dedicated Consultant, Foundation year 2 Doctor, Specialist Care of the Elderly Band 7 and Band 6 Nurse, Band 6 Physiotherapist and Occupational Therapist, Pharmacist and Social Worker with rapid access to SALT, Dietetics and Mental Health for Older Adults.
