Senior Locality Pharmacist Band 8a

In Croydon, health and social care providers and commissioners have agreed a whole system approach for the delivery of health and social care. The One Croydon Alliance partnership is a joined-up service for people needing health and social care support, from help with leading a healthier lifestyle through to avoiding unnecessary hospital stays and supporting people in their own homes and community.

The main service developments are the creation of an integrated reablement and rehabilitation service, and an Integrated Community Networks (ICN+), improve multiagency working in the community, built around the GP networks and strengthening MDTs at practice level .

The ICN+ Medicines Optimisation Service (ICN+ MOS) is six specialist clinical pharmacists working across localities in Croydon. They support vulnerable and housebound patients with long term conditions, to use medicines safely and appropriately and to manage medications independently.

ICN+ core team members include GPs, pharmacists, community nurses, social workers and personal independence coordinators. Teams attend regular multi-disciplinary "huddles" with GP practices and other health and social care providers, to identify high risk patients and develop care plans.

Main duties of the job

Provide expert leadership and specialist professional advice on medicines management in designated locality. Be responsible for developing and maintaining a service, supporting medicines issues for patients in domiciliary settings. Undertake reviews and support patients with their medicines. Make recommendations and support development of frameworks to ensure safe management of medicines via strategy, policy and training within the Trust and ICN+. Supervise band 7 and band 6 junior pharmacists, technicians and pre-registration graduates, as needed. Attend & contribute to ICN+ and MDT meetings e.g. GP huddles, incl. identifying individuals suitable for discussion. Have knowledge of other services linked to ICN+ and partner organisations enabling referral and signposting.

Be responsible for developing systems to collate data, case studies and any information for regular reports to be generated. Be able to manage time effectively, be self-motivated and have the ability to prioritise based on the needs of the ICN+ and individual patients.

The post-holder will mainly work independently in a designated locality ICN+, however line management and professional supervision will be provided through the Lead Pharmacist responsible to the Chief Pharmacist in Croydon ICB. The post holder will need to be aware of guidelines and workplan areas of the Medicines Optimisation Team so they can support and promote co-ordinated messages.

Supervise Junior and rotational pharmacy staff.

Essential

  • Pharmacy Degree MPharm or equivalent.
  • Candidate must be Registered with GPhC and eligible to work in the UK.
  • Postgraduate Diploma in Clinical Pharmacy or equivalent.
