The post holder will contribute to the specialist nursing service for chronic rheumatology patients.
The post holder is a clinical nurse specialist within a team providing specialist care to rheumatology patients, that is sensitive to patients' needs and based on researched clinical practice. The post holder will work collaboratively with other staff, and will take responsibility for the assessment and monitoring patients within the outpatient clinics. Although the post holder will focus primarily on the condition of rheumatoid disease, other chronic rheumatological conditions have access to this service and the post holder will maintain appropriate knowledge and skills.
Main duties of the job
We are seeking an enthusiastic, driven nurse with excellent clinical skills to join the Department of Rheumatology.We are looking for an individual that is highly motivated, able to work effectively within in a large multidisciplinary team and also to use their own initiative within their scope of practice. The successful candidate will have responsibility for the management of patients with complex rheumatologymulti-system health needs,and will promote the service and deliver high quality care that is evidence based and reflects current best practice. They will work collaboratively within the Rheumatology team and be visible and accessible to patients, carers and staff. The post holder will provide specialised advice, education, training and support for patients and support the Lead nurse with the development of care pathways and nurse led initiatives to enhance services delivered. For this role you must demonstrate that you are reliable and flexible. The candidate will be responsible for the assessment and treatment, health education and support to patients to manage their conditions in clinic, via a telephone advice line and virtually using tele-medicine.
