Job summary
To provide and manage expert specialist clinical practice in the borough of Croydon forcommunity/housebound patients with diabetes.
To be a source of expertise, knowledge and support, upskilling community health care professionals through joint and virtual clinics.
To develop evidence based clinical nursing in line with current research and guidelines. To ensure patients and their families are fully informed and supported in the management of their diabetes care.
To work with the wider multidisciplinary team in a proactive way with the aim of preventing hospital admission, supporting early discharge and reduced GP contact.
To work within the community based services to provide structured education for people with diabetes.
To provide nurse led clinics in community locations to support the management of patients with diabetes. This will include initiating diabetes medication and monitoring patients ongoing glycaemic control andtitrate medication.

We are an Integrated Diabetes Service which provides seamless, high quality care for the people of Croydon.
You will provide clinical / education support for the Primary Care Team, patients with both type 1 and type 2 diabetes and their carers.
We offer education/support to help with personal development, Apprenticeship offerings and other new development opportunities.
You will have the opportunity to be part of the specialist services which include:
- Antenatal care - Diabetes in pregnancy Clinics
- Complication management, including foot,
- Renal Clinics
- Transition and young persons' clinics
- Insulin pump Clinics and the management of problematic hypoglycaemia.
- Libre clinics and other Diabetes technologies
- Psychologist & DSN Joint Clinics
We are seeking to recruit a capable Diabetes Specialist Nurses who have a passion for improving the care of people with diabetes, excellent communication skills and enjoy working as part of a team.
The role will include working closely with colleagues in hospital, community and primary care to ensure that the people of Croydon receive high quality integrated care.
A clean driving license and use of a car for work purposes is desirable.
