Job summary

The postholder will manage a portfolio of projects aligned to strategic priorities within the Population Health & Care programme, overseeing the full project lifecycle from engagement and planning through to delivery and closure. They will ensure consistent project processes and act as the first point of contact for all enquiries, providing advice and guidance to stakeholders.

A key aspect of the role is stakeholder management across all levels, including board members and clinical staff, ensuring requirements are clearly understood and met through data-driven tools and analysis. The postholder will proactively engage partners to support collaborative, sustainable approaches that respond to Lewisham's evolving health and social care needs.

They will also be responsible for scheduling development activities and allocating resources, such as data analysts, to balance demand and capacity while ensuring delivery of measurable benefits to target populations. The role supports value realisation across the wider health system, including primary, secondary, community, and mental health services.

The postholder will manage multiple complex, cross-organisational projects, addressing resource constraints, mitigating risks, and contributing to continuous improvement of team processes.

Main duties of the job

The postholder will lead portfolio management and provide support across a range of transformation projects, developing and maintaining strong relationships with senior stakeholders, including directors and clinical staff, to deliver measurable outcomes for target population cohorts. They will develop and manage programme and project plans in line with Lewisham PHM processes, using the IPO (Input-Process-Output & Outcome) framework to ensure timely delivery of milestones, benefits, and objectives.

The role involves proactively managing risks, issues, and dependencies using tools such as Asana, Jira, or Excel, ensuring minimal disruption to delivery. The postholder will deputise for the Associate Director where required, including stakeholder engagement, escalation management, and liaison with system partners and suppliers.

They will design and manage multiple complex projects in a fast-paced environment, leading multidisciplinary teams and ensuring clarity of roles and responsibilities. The postholder will work closely with stakeholders to define requirements, analyse information, and optimise solutions, ensuring clear documentation and effective scope management.

Additionally, they will support the development of operational pathways, case management processes, and business cases, driving innovation and efficiency in line with PHM strategic goals.

  • Educated Masters degree or equivalent experience
  • Approved Project Management Qualification (Prince 2/MSP) or extensive experience in a similar role in a similar environment.

  • Formal training in population health management or relavant
