Patient Discharge Navigator - Medical Admissions Unit

Job summary

Are you looking for a challenge which requires flexibility, forward thinking and drive? An exciting opportunity has arisen for a motivated enthusiastic individual to join the team on Chestnut ward our Medical Admissions Unit as a Patient Discharge Navigator. This role will help ensure a smooth and safe transition by coordinating discharge plans, accessing necessary resources, and providing information and support to our patients, their families and carers.

Main duties of the job

We are seeking a committed, dynamic and forward-thinking individual, who will join a well-respected,established team on Chestnut ward to support the flow of patients on and off the ward.

Effective communication, strong collaborative approaches and the ability to engage the entire multidisciplinary workforce within an ever changing, ever challenging environment is essential.

You will be expected to optimize patient clinical pathways, enhancing patient experience, and reducing patient length of stay.

You will need to work as part of a wider team but also be able to work autonomously, prioritising your workload and communicating delays and concerns in a timely manner.

Work as part of the multidisciplinary ward team, gaining an insight and knowledge onto the needs of all the patients on their allocated wards.

Support patients, their families and carers to ensure that assessments and discharge plans are fully understood and agreed.

Attend the daily board meetings to proactively identify patients who are ready for discharge and transfer to a backend medical bed.

Essential

  • oEnglish and Maths at GCSE grade C or above.
  • oCourses/ further study to demonstrate evidence of personal development
  • oNVQ 4 or equivalent experience

Desirable

  • Knowledge of medical terminology

Experience

  • oPrevious experience of working with the public/ and or patients in a healthcare environment.
  • oExperience of interaction with all levels of staff
  • oPrevious administrative and organisational experience

  • Experience of managing and prioritising own workload, and working to deadlines.

  • oExcellent written and verbal communication skills
  • oExcellent organisation skills with a meticulous attention to detail
