you will be working as part of a Specialist Team providing expert nursing/midwifery practice and cost-effective service to patients requiring routine and emergency care. To work primarily, but not exclusively, in the resuscitation area to provide, develop and coordinate high quality care to patients attending with emergency and trauma care needs. As a senior nurse or paramedic, you will hold expert clinical skills in patient assessment, recognition of the deteriorating patient, effective escalation and/or handover as well as initial patient management of acute illness and trauma. To develop skills of history taking and physical assessment to direct appropriate intervention. To demonstrate specialist emergency care and trauma expertise underpinned by theory and incorporating evidence-based practice, evidenced by completion of appropriate work-based learning competencies To maintain accountability and demonstrate skills in undertaking assessment, planning, implementation and evaluation of patients presenting with emergency care needs without supervision To be able to conduct a rapid history and A-E patient assessment, documenting this appropriately, with the ability to effectively hand a patient over to the ED Doctor or Advanced Clinical Practitioner (ACP) in a thorough and timely manner To work with the clinical team to ensure that all patients have equitable access to investigations, treatment and follow-up care. To work with the clinical team to develop best practice in line with national guidance (such as NICE/ RCEM). To work with the multidisciplinary team to deliver quality standards in line with evidence based practice. To demonstrate specialist knowledge and skills and share this with others to positively influence patient care. To provide a comprehensive nursing service for patients with conditions within the specialty. To use clinical judgement and skills in decision making to manage this caseload of patients. To review and modify planned interventions in response to the needs of emergency care patients acting independently where appropriate and seeking advice where required To order and where appropriate obtain appropriate diagnostic investigations. Such as x-rays, article blood gases and laboratory tests, to assist in managing episodes of care To identify deteriorating patients and take the appropriate action to manage the situation, liaising with interdisciplinary team as appropriate. To obtain valid and effective consent from patients prior to any investigations and treatment. To provide leadership for the nursing team in the resuscitation area to ensure a high standard of patient care, providing support and supervision as required. Providing appropriate escalation if care needs are not being met, to the Nurse-In-Charge and/or Doctor/ ACP in charge of patient care in that area. Working closely with wider Trust multi-professional teams such as critical care and outreach. To complete and administer a range of medications under Patient Group Directives (PGDs) once completion of training, to aid patient care Partnership: To work with patients and carers to assess their specific educational needs to ensure that they receive full information about their condition. Partnership: To provide patient education and appropriate information leaflets. Partnership: To provide a seamless service for patients across an integrated pathway of care. Partnership: To refer patients to other appropriate specialists to ensure the needs of the patients are met. Partnership: To involve patients/carers views in developing a vision for the future of the service by undertaking patient questionnaire reflecting their experiences and outcomes. To keep accurate and complete records of activities and communications consistent with legislation, policies and procedures. To review and maintain standards of documentation in accordance with NMC and Trust standards for records and record keeping.Organisational management To coordinate the admission / reception of new patients and arrange care of their property in accordance with Trust policy To work in close liaison with the multi-disciplinary team across the pathway in order to meet the needs of patients and their famillies, and to aim for seamless care. To work with the team to implement recommendations from National audits and benchmarking. To contribute to any audits requested by the Trust, Commissioners or Nationally.
