Job summary

To work as part of the Emergency Department and Same Day Emergency Care service as a trainee ACP.

The post holder will work clinically developing a portfolio of practice as per the Royal College of Emergency Medicine. They will undertake and be required to pass an advanced Clinical practice masters within a London University.

Main duties of the job

The Trust has two EDs; one on the Queen Elizabeth (QE) site and one on the University Hospital site (UHL); which are some of the busiest within London seeing in the region of 140,000(QE) and 119,000(UHL) patients annually. The team on both sites is made up of a range of senior nursing and medical roles, who are supportive of this role. We have a dedicated ACP lead Consultant that covers both sites.

The trainee ACP role will provide expert patient assessment and management as outlined in the job description in all areas of the department and will be expected to work on both QE and UHL sites during the trainee phase. The trainee will also support and work within the Same Day Emergency Care (SDEC) service .

The role will have a 24 hour 7 day rota allocation.

Candidates are required to have significant acute care experience and are welcomed from both an appropriate nursing or allied health care background. You must be dedicated to the requirements of the training phase as outlined below. The trainee ACP will be a senior member of the ED team and as such would be required to demonstrate self-motivation, dedication to high quality of care and clearly evidence their own personal and professional development.

Job description

Job responsibilities

This job will encompass the skills of assessment, examination, diagnosis and treatment within an agreed scope of practice throughout the Emergency Directorate (ED). The post holder will under supervision assist in the safe referral and discharge of patients with undifferentiated and undiagnosed presentations in designated areas of the ED and Same Day Emergency Care

To deliver and participate in clinical care of patients attending the ED

Provide a level of advanced and comprehensive nursing assessment, based on a specialist body of knowledge, under supervision.

Direct responsibility for the management of patients within own case load and for the supervision of junior members of the multidisciplinary team

To appropriately assess, examine, investigate, diagnose and treat patients, resulting in the safe management and appropriate referral or discharge of patients with undifferentiated and undiagnosed presentations within the boundaries of the role as a trainee.

Order and interpret investigations, laboratory tests, x-rays and act on results as appropriate, with supervision as required as a trainee

Initiate drug therapy/medication within the parameters of agreed clinical guidelines and in accordance with present legislation supply and prescribing of medicines.

To contribute to the development and improvement of systems and processes that facilitates patient flow.

Maintain own clinical development by keeping abreast of new treatments and technologies within emergency and unscheduled care.

To improve the quality of the patient experience by identifying and meeting the individual clinical needs of patients.

To rigorously review all aspects of the patients plan of care and identify and address issues that may result in a sub-standard service.

Contribute to the review and development of integrated care pathways and clinical guidelines to ensure a standardised approach to emergency care.

Under supervision practice with confidence and competence within your professional guidelines and code of conduct (NMC / HCPC)

To ensure that accurate documentation and records of patient care are kept.

To assist in the development of assessment systems, processes and tools within the ED and adjacent clinical/assessment areas.

Ability to adapt specialist clinical knowledge and skills to different clinical settings and influence service delivery and patient care

Actively implement effective systems and processes for infection control management and relevant audit within their clinical area.

Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.

Communicate patients conditions verbally and in writing to other members of the health care team and contribute to the decision making processes in patient management. Acting as an advocate for patients and colleagues.

Ensure awareness of sources of support and guidance e.g., patient liaison service, and provide information in an accurate format to all patients.

Be able to anticipate barriers to effective communication and take action to improve communication.
