Job summary

The EoLC Discharge Nurse will: Support others to, and facilitate timely, coordinated and appropriate discharges for those patients at University Hospital Lewisham who are at the end of life, regardless of diagnosis, to their preferred place of care wherever possible. Support patients and their carers/families in having conversations about advance/ future care planning / access and application to CHC funded care and discharge planning. Establish and develop relationships with those involved in delivering health and social care to patients at the end of life in all settings and work collaboratively to achieve appropriate care for the patient. Develop efficient protocols and pathways with appropriate information which prioritise palliative care patients and address the particular needs of this vulnerable group of patients Actively disseminate best practice through example, education and training, professional development and learning, participation in audit. Work cohesively as part of the multiprofessional team.

Facilitate ward staff to initiate the discharge planning process through effective education, training and leadership.

Work closely with primary care colleagues in continuing health care/ integrated care boards to design appropriate packages of care.

To act as a resource and advocate for patients and their relatives/carers.

To participate in audit/research.

Main duties of the job

Provide a high standard of holistic and individualised patient care to patients with palliative care needs by:

Acting as a role model for others to emulate, exhibiting high standards of professional behaviour and competence and providing leadership, support, advice and education to colleagues caring for people at the end of life in all settings.

Making assessments of patients and carers, identifying their psychological, physical, social, practical and spiritual needs in accordance with palliative care philosophy and demonstrating and applying knowledge of the options for discharge in order to develop a discharge plan to meet their needs.

Acting as a reflective practitioner, applying evidence-based practice and critical thinking to all practices and processes of care.

Working collaboratively with other professionals involved in the patient's care in any setting, to provide a co-ordinated, high quality service. This will involve working with statutory, independent, charitable and voluntary agencies in health and social services.

Prioritising own workload according to the day to day needs of the service.

Providing guidance, support, advice and information to patients and their family / carers throughout their illness. Know when to refer on to other professionals for additional guidance, support, advice and information.

This post has been developed to complement the work of the complex discharge and palliative care team members to improve and expedite discharge of patients approaching end of life from UHL.
