Job summary

An opportunity for a Cardiac Nurse Specialist on Secondment has risen in the Cardiac Rehabilitation service, which values professionalism, teamwork, and collaborative learning. A small, friendly, and supportive team dedicated to consistently improving and enhancing the patient experience and the quality of the treatment.

The post holder will have the ability to develop and provide a continuity of care and support to patients and their families, across the Cardiac Rehabilitation process. The roles include utilising knowledge and skills to support, educate, and inform patients and their families after a cardiac event, reviewing the coronary risk factor profile to counsel and educate patients to modify their lifestyle, ensuring that secondary prevention strategies are put into practice, and making sure patients are appropriately referred to other specialists as needed.

Main duties of the job

  • To support the Band 7 Nurse Specialist and Exercise Physiologist in the development of new initiatives and coordinate and maintain the Cardiac Rehabilitation Service in the absence of the Band 7 Specialist Nurse.
  • Contribute regularly to quality monitoring of the service and assist with the national audit.
  • The post holder will coordinate and participate in a national audit to evaluate and develop services, as well as monitor patient experience.
  • To work as part of a multidisciplinary team, understanding of the commissioning guide
  • to provide a comprehensive cardiac rehabilitation service.
  • Provide patients with an individualised programme of care to facilitate physical, psychological, and emotional recovery.
  • Liaise and contribute to Phase IV community cardiac rehabilitation programmes to ensure continuity and quality of care for cardiac patients.

All the following are to be carried out while working in close collaboration with members of the multi-disciplinary team led by a consultant. To demonstrate clinical expertise within the speciality and work autonomously, seeking guidance/support when necessary.

  • To assist with the development and expansion of the current cardiac rehabilitation service providing evidence-based management of cardiac patients.
  • To work as part of a multidisciplinary team to provide a comprehensive cardiac rehabilitation service.
  • To provide cover in the absence of the other cardiac nurse specialists.
