Job summary

Applications are invited for a Clinical Fellow post in Regional Anaesthesia to join the Department of Anaesthesia at University Hospital Lewisham. The appointee will join a friendly and active department with an excellent reputation for teaching and training. The appointment is for a fixed 6 month contract, although previous Fellows have extended this to a year subject to satisfactory performance and reviews during this time.

The regional anaesthesia fellow post offers regular dedicated orthopaedic and trauma lists, where regional anaesthesia is routinely performed. The department has a consultant body that includes 6 EDRA diplomates, and regional techniques are embedded into our ERAS lower limb joint protocols, upper limb lists and foot surgery lists.

The anaesthetic department supports regular orthopaedics and trauma, obstetric, general surgery, gynaecology, paediatric day surgery, urology and ENT lists, working across 11 theatres. There are close links with the Critical Care team although this is staffed on a separate rota.

Applicants must be fully registered with the General Medical Council and will be required to meet the essential criteria as described in the person specification. The applicant should have gained their initial assessment of competence in obstetric anaesthesia.

Main duties of the job

To help ensure the smooth running of the service provided by the department of anaesthesia, providing high quality care to patients.

To work with a group of SAS doctors, clinical fellows and anaesthetic doctors in training grades to ensure provision of a safe and effective out of hours anaesthetic service covering a dedicated obstetric tier and a theatres/emergency tier. To provide cover for colleagues during periods of annual/sick leave.

To participate in educational and audit meetings and meet CPD requirements.

To run sonoanatomy teaching sessions to trainee and consultant colleagues (as part of ongoing "sonoclub")

To support the hospital's clinical governance strategy and to participate in clinical audit / quality improvement.

To continue training and achieve educational objectives

The UHL anaesthetic department has a strong interest in regional anaesthesia: six members of the consultant body have the European Diploma in Regional Anaesthesia, whilst others have taught regionally and nationally on areas such as neuraxial ultrasound and paediatric regional anaesthesia. Regional anaesthesia is incorporated into ERAS elective arthroplasty pathways (for both hips and knees) and day case upper limb lists rely on regional blocks to enhance patient experience and facilitate day-case turnaround. Trauma cases, major colorectal cases and paediatric day case surgery (e.g. inguinal hernia repair) provide further opportunities for regional anaesthesia experience.

We liaise closely with our Acute Pain Service colleagues who are keen to use regional anaesthetic techniques where appropriate for in-patient pain issues. A nerve catheter service has been introduced, with much of this work led by previous regional fellows. Patients with rib fractures and laparotomy patients now benefit from access to continuous nerve block techniques. The department has also reinstated a regular sonoanatomy teaching session (Sonoclub), to provide updates and hands-on ultrasound teaching to both trainees and the consultant body.

The appointee to this post can expect to have a minimum of 2 lists / week (averaged across the rota) where they will be assigned supervised lists appropriate for gaining regional anaesthesia experience. In addition they will be expected to participate in the organisation and running of the Sonoclub, and become involved in at least one quality improvement initiative relating to regional anaesthesia.
