Job summary
- To work as a lead clinical nurse specialist and effectively deliver an expert nursing service as part of the palliative care team working on the Lewisham Hospital site
- To contribute to the management of a caseload of patients with palliative care needs
- To ensure that patients and their families and fully supported and informed
- To assist in the development of evidence based clinical nursing practice in line with current research and guidelines
- To maintain clear lines of communication within palliative care to ensure cohesive multidisciplinary management of patients
- To link with other palliative care providers and primary care to facilitate the care of patients transferring to community settings
- To act as a role model for other nurses and support the delivery of palliative care education within the Trust
- To engage in Team and Trust Governance

Main duties of the job
- To lead the team in providing a specialist nursing service to people identified with specialist palliative care needs
- To manage and work with the team in delivering responsibility for a daily patient caseload ensuring that all patients have a comprehensive plan of care which reflects the assessment undertaken and incorporates issues identified and recommendations made
- To accept responsibility for ensuring clear documentation in patients records and any other relevant trust systems
- To support a seamless service through the development of enhanced multidisciplinary (MDT) processes and communication
- To ensure that all new patients referred to the service are discussed at a weekly MDT and outcomes recorded in patients clinical record, to attend and actively participate in the MDT
- To work with local community services to support the development and maintenance of services for patients with specialist palliative care needs
- To ensure that all patients are fully informed to support informed consent
- To support agreed Trust processes for patients who lack capacity to ensure best interests are upheld
- To ensure that information is provided in a format that is acceptable and understandable for the individual patien

- To work with the wider MDTs, PDNs and link nurse groups to support the ongoing implementation of the Trusts Priorities for Care of the Dying Patient
- To participate in the development of strategies and clinical protocols to support standardisation of services and equity of access for those referred to our service

- To work with ward and discharge teams to ensure timely and appropriate arrangements are in place for discharge/follow up in community
