As a result, the number of older people in England is growing significantly and this rate of growth is projected to speed up over the next 20 years. More than one in five of us are already over 60, and the number of people over 60 is expected to increase from 14.9 million in 2014 to 18.5 million in 2025
This is an exciting opportunity for an experienced clinical pharmacist to lead and develop the pharmacy service to the directorate of Frailty and Older Adults at the Trust. The post-holder will work together with the Principal Pharmacist for Medicine and be responsible for expanding and developing the pharmacy service across the Trust in collaboration with other lead pharmacists.
The role will also offer the exciting opportunity to become an Advanced Clinical Practitioner and utilise independent prescribing skills by being an integral member in consultant-led frailty post take ward rounds and following up discharged patients in outpatient frailty clinics.
Main duties of the job
You will be expected to lead and mentor a team of pharmacists and pharmacy technicians, implement policies and procedures to ensure safe and effective medication management for our older patients, ensure there is safe and effective medication management for our older patients, provide education and training to staff and patients on safe and effective medication use and to participate in research and quality improvement initiatives to improve patient outcomes.

- Be a fundamental member of the Frailty Team at Lewisham and Greenwich NHS Trust, through contribution to post take ward rounds, outpatient frailty clinics and participation in MDT and governance meetings
- To support the division in managing their drugs budget and act as the point of liaison for any service developments that involves medicines or pharmacy services.
- To provide operational and clinical support to the pharmacy department as dictated by the needs of the clinical pharmacy service. This includes supervision, training and support to junior pharmacists, pre-registration pharmacists and other pharmacy staff as necessary.
