Job summary
An exciting opportunity has arisen for an enthusiastic project manager to lead on a Stroke improvement project for the South East London (SEL) Integrated Stroke Delivery Network (ISDN). The SEL ISDN sits within the London ISDN which has links into the ISDN and is tasks with improving stroke standards locally, regionally and nationally respectively.
The successful post holder will lead on a proof-of-concept project aimed at improving the flow of on patients through the Sentinel Stroke National Audit Programme (SSNAP) and ensure maximum capture of data throughout the stroke pathway. They will link in with other Stroke Services in London and Nationally to ensure the optimum model is being used in SEL.
The role will support stroke services within SEL to develop and integrate change, improve their ability to use SSNAP data to support long-term development.
The post holder will work alongside the SEL Rehab Lead for Stroke and all parts of the Stroke Pathway from to achieve the project objectives and support innovation and change within stroke services in SEL.
Main duties of the job
Key responsibilities will include:
- Responsible for the delivery of this SSNAP Coordinator Proof-of-Concept Project.
- Set-up and delivery a SSNAP coordinator service for SEL to ensure maximum capture of data through the national audit programme.
- Understand the specific challenges to prompt and accurate data capture in all parts of the Stroke Pathway in SEL, and help to define the new service, create training resources and processes needed to ensure high quality data inputting and efficient data entry.
- Manage a team of administrative staff as directed by the general manager.
- Feedback to all key stakeholders on the progress, outputs and outcomes of the project.

Key Areas & Performance:
Post holder will work with Project Lead, Hyper Acute Stroke Units, Acute Stroke Units, Community Teams, 6 Month Stroke Review teams in South East London (SEL) and other members of the ISDN on:
- Set-up and delivery a SSNAP coordinator service for SEL to ensure maximum capture of data through the national audit programme.
- Understand the specific challenges to prompt and accurate data capture in all parts of the Stroke Pathway in SEL, and help to define the new service, create training resources and processes needed to ensure high quality data inputting and efficient data entry.
- Regulatory, national and NHS requirements ensuring alignment with the organisational strategy.
- Producing detailed business plans including milestones, delivery dates, KPIs, risk assessments and quality impact assessments for submission and approval.
- The delivery of (or as the need arises work actively to deliver) the identified outputs and KPIs.
- Monitor, interpret and quality assure progress against deliverables, that often require adjustments specifically in relation to the ISDN and national stroke agenda.
- Providing an appropriate balance of challenge and support to the Project Lead, ISDN, HASUs, ASUs, Level 2b and Level 1 rehab beds, Community Stroke Rehab Team and 6-month Stroke Review Teams in the development and delivery of SSNAP Coordinator Team and in the tracking of activity against plan.
- Producing reports detailing progress against deliverables.
- Working with the Project Lead, HASUs, ASUs, Level 2b and Level 1 rehab beds, Community Stroke Rehab Team, 6-month Stroke Review Teams and other ISDN members to identify the barriers and challenges to accurate SSNAP data collection across the Stroke Pathway and development the necessary processes to improve the percentage of Stroke patients captured on SSNAP and agree these with key stakeholders.
- Integrating with various support services i.e. Finance / Human Resources / Business Intelligence / Information Technology to ensure all aspects of the project are addressed to enable successful completion of the project.
- Ensuring that outputs and benefits defined within the project brief are delivered on time, and to the required standard of quality.
- Managing data collection throughout the Stroke Pathway ensuring that no work is either overlooked or duplicated considering risks associated with interdependencies and links between the different stakeholders, ensuring they are managed to a successful outcome.
- Scoping changes and updates to data collection and inputting procedures to deliver the required service improvement.
- Developing and implementing the data inputting pathways using learning from national best practice to inform changes to systems and processes.
