Job summary

An exciting opportunity has arisen for a Population Health Portfolio Manager to join the Population Health Care team at Lewisham and Greenwich NHS Trust (LGT), where they will play an integral part of the Population Health & Care programme team.

Main duties of the job

  • Develop and maintain collaborative relationships with a broad range of senior system stakeholders to define and implement transformation projects
  • Produce and maintain programme and project plans for work requests to ensure timely delivery of agreed milestones and objectives
  • Identify and manage portfolio risks, issues and dependencies to ensure successful delivery of projects and minimise disruption to the programme
  • Deputise for Associate Director where appropriate, including but not restricted to stakeholder engagement, escalation meetings, working group facilitation, product demonstrations and liaison with Oracle Cerner
  • Lead and engage local project and clinical working groups alongside Population Health Data Analytics workstream, ensuring all members understand their roles and responsibilities

Job responsibilities

Portfolio Management and Support

  • Develop and maintain collaborative relationships with a broad range of senior system stakeholders at all levels including director and clinical staff, to clearly define and implement defined transformation projects which will enable the realization of clear measurable outcomes for the target Population cohort.
  • Develop and maintain programme and project plans for work requests to ensure timely delivery of agreed milestones and objectives, including achievement of identified benefits and outcomes.
  • Proactively identify and manage portfolio risks, issues and dependencies to ensure successful delivery of projects and to minimize disruption to delivery.
  • Deputise for Associate Director where appropriate, including but not restricted to stakeholder engagements, escalation meetings, working group facilitation, product demonstrations & liaison with Population Health Software supplier (Cerner / Oracle).
  • Design, implement and manage a broad range of complex projects, which may take place simultaneously in a changing and fast-paced environment
  • Lead and engage local project and clinical working groups, ensuring all team members understand roles and responsibilities and are confident to perform them effectively, making decisions and / or taking corrective action when required.
  • Work with stakeholders to facilitate the articulation of project requirements and clearly define the outcomes and benefits to be enabled by the project. Identify and agree key requirements of complex matters; interpret and analyse information to optimize the design of solutions for delivery and implementation. Ensure these are clearly documented in the Project Request Document Template and manage any changes to scope including carrying out impact and feasibility assessment.
  • Design and scope projects, benefits identification and produce business cases, including extensive research into new ways of working to improve efficiencies across the Trust

Administration, Governance and Project Documentation

  • Facilitate and advise on the design and development of a governance structure, ensuring appropriate governance arrangements are in place to facilitate engagement, delivery and decision-making between stakeholders
  • Ensure robust and sustainable monitoring and tracking of projects and delivery progress
  • Develop, implement and improve programme documentation for team-wide use
  • Plan, organize and facilitate project meetings

Communication and Stakeholder Management

  • Develop, build and maintain effective partnerships and relationships with a range of stakeholders
  • Communicate with external bodies and be aware of sensitivities and political considerations
  • Communicate confidently with people at all levels, across the Trust and associated external partner organisations
  • Make decisions on a range of complex and sensitive issues where there may be more than one course of action, using own expertise and within responsibility level and remit

Responsibility for Information Resources

  • Implement, improve and maintain a knowledge management system that will enable Team Members to share and organize information, processes and key insights to facilitate continuous improvement and best practice within the team
  • Source, understand and distribute highly complex information associated with service delivery, project management and business development, both internally and externally
  • Utilise and actively support use of bespoke technology to monitor service progress, delivery and quality
  • Contribute to the development, implementation and review of new and/ or existing information management systems/ services as required, to support team growth and change
  • Manage and produce complex spreadsheets and reports as required

  • Educated Masters degree or equivalent experience
  • Approved Project Management Qualification (Prince 2/MSP) or at least 5 years experience in a similar role in a similar environment.

Desirable

  • Formal training in population health management and familiarity statistical data analysis
  • Familiarity with data coding (e.g. SQL) and developing data queries.

Experience

Essential

  • Experience in a project or portfolio management role
  • Experience of maintaining activity to deadlines so project progress remains on track
  • Experience working as part of a dynamic fast paced team
  • Examples of leading on change management/service improvement within the NHS and/or social care
  • Practical experience of managing multiple successful projects, including the development of complex project management frameworks, monitoring and review processes
  • Proven successful planning and organisation experience
  • Performance management, including monitoring of delivery to timescales
  • Experience handling and processing sensitive data and compliance with Information Governance and Data Protection principles in a multi-organisation environment.
