Job summary
This is an exciting opportunity to work in the newly created Start for Life Team. In conjunction with the team the HVAP will contribute to the assessment of health needs and delivery of appropriate interventions to meet needs of families and C&YP as per the defined clinical pathways of the Start for Life Programme.
They will undertake duties as delegated by the Programme Lead and or Specialist Health Visitor, which includes child health promotion and surveillance, predictive guidance support and advice to families with children under 5 years of age. They works as part of the multi-disciplinary health care team, liaising and working with statutory and voluntary agencies to promote the health and wellbeing of the local population as appropriate.
Main duties of the job
- To work closely with the Start for Life Team in the planning and delivery of health promotion activities and public health initiatives. The Community HVAP will take on responsibilities in areas relevant to their competence and skills.
- Work in conjunction with the team to formulate care plans for a programme of intervention to be carried out either at home or clinic settings.
- To work within the team to meet the specific health needs of individual children and families. This may involve home visiting.
- To observe and report any changes and concerns to the Health as identified during their contacts with families.
- To carry out child health promotion programme in the clinic, at home or pre-school/Early Years settings
- To provide health promotion/ education in a range of settings including groups and 1:1 situations, using a range of media to get (evidence-based) health messages to the population, with support from the Health First. To develop appropriate teaching material/resources for parents.
