Job summary
We are seeking to recruit an enthusiastic and well-motivated individual to join the Hypertension service as a Patient Access Coordinator. The successful applicant will be able to work effectively under pressure, use their own initiative, and manage competing priorities within a busy clinical environment.
Excellent organisational and time-management skills are essential to ensure the smooth coordination of patient access, appointments and pathways of care. The post holder will play a key role in supporting patients and their families throughout their care journey, providing a professional, responsive and compassionate service at all times.
The Patient Access Coordinator will act as a central point of contact for patients, carers, clinical teams and other bodies such as GPs and Pharmacist, ensuring accurate information, timely communication and effective pathway management. A strong commitment to delivering high-quality patient care and maintaining patient confidentiality is essential.
The post holder will be expected to consistently uphold Guy's and St Thomas' NHS Foundation Trust Values and to work in full accordance with Trust policies, procedures and governance standards.

Main duties of the job
- Supported the operational delivery of specialist hypertension outpatient clinics, ensuring effective coordination of clinics, workforce, and patient pathways.
- Managed clinic templates, capacity, and waiting lists to support timely access to care and compliance with RTT and local performance targets.
- Worked closely with consultants, specialist nurses, and administrative teams to ensure smooth patient flow and high-quality service delivery.
- Monitored activity, outcomes, and DNA rates, contributing to service improvement initiatives aimed at reducing delays and improving patient experience.
- Supported pathway development and service transformation aligned with Trust objectives and national hypertension guidelines.

Ensure that the vetting process for referrals has been followed in line with trust and departmental guidelines.
Be responsible for the scheduling of outpatient and all associated appointments according to clinical urgency and length of wait in accordance with departmental procedures and Trust Policies.
Liaise with patients prior to their hospital visit to ensure that a mutually convenient date and time is agreed.
Ensure that patients are appropriately reminded of their appointments. This may include sending reminder SMS or contacting patients by telephone, up to 2 days before their visit to re-confirm attendance in line with departmental protocols.
Ensure that departmental standards are met for patients receiving notification or acknowledgement of their visit.
Provide a courteous and efficient point of contact for patients over the telephone.
Ensure that each patient is treated as an individual and their individual needs are met (i.e.: appropriate support is in place for their appointment, such as interpreting support).
