Job summary

The Integrated Care Transformation Team (ICTT) work in close collaboration with the ISM Clinical Group, Partnership Southwark, Lambeth Together and other partners in the SEL ICS to lead and deliver transformation and quality improvement programmes. ICTT are a place-based transformation team driving systemic integrated care for both Boroughs. The team is made up of programme and project managers, service development managers, a data analyst, a primary care liaison manager and business support. The team's portfolio consists of improvement programmes of work to support resilient communities, optimise efficient transfers of care and embed population health principles. The Head of Portfolio will manage the team and its delivery of partnership programmes.

The Head of Delivery for the ICTT will be responsible for leading, managing, delivering and directing the Integrated Care and Transformation team and their work programme. The relationships with internal and external partners will be key for this role; to deliver the Trust and the system priorities associated to integration and the Trust strategy associated to the Health of our Population.

Main duties of the job

The post-holder needs to be adept at working across departments, levels of the organisation and across the system to lead, inspire, design, and drive transformation and programme management. They will have a high level of autonomy, self-drive, and an ability to direct change initiatives which will achieve the aims of the programme and organisation.

The post holder will lead on building transformation and programme management capability within their programme. Acting as a subject matter expert, they will advise executives and senior leaders to design and lead work to embed transformation through role modelling, training and mentoring. A key focus will be ensuring programmes and teams can clearly articulate the benefits of their work, particularly in relation to system change, improving patient experience and delivering on key performance indicators.

ICTT Head of Delivery:

Lead the ICT team and line manage the Senior Programme Managers. Provide support to them and their teams to deliver against set objectives.

Develop management and operational processes to govern, monitor, track and report on the activities of the Delivery function.

Design, facilitate and lead ICT team meetings and regular meetings with other stakeholders including external agencies and partners.

Develop communication tools which serve to engage employees and create transformation and improvement opportunities and conversation at all levels in the Trust.

Support the ICTT prioritisation process for transformation and improvement opportunities that sit in the pipeline of initiatives.

Work with others in ICTT and wider transformation teams in GSTT and in the wider system on pre-empting problems and misunderstanding in relation to work of the ICTT.

Provide senior level transformation, change management and programme management expertise

Staff Management:

Provide senior leadership within the ICTT and influence transformation and programme management capability across the organisation

Provide day to day leadership of the team and be a source of subject matter expertise on transformation implementation and programme management across the Trust

Responsible for conducting staff annual performance appraisals and reviews

Manages budget for own area of work and multi-stranded project workstreams to ensure best value for money, including signing off invoices, monitoring expenditure and providing budgetary setting and reports.
