If so, you should apply for the role of Joint Formulary and Medicines Pathways Pharmacist. This role will require you to combine your well-developed clinical pharmacy knowledge with advanced critical appraisal skills. You will be responsible for assisting in delivery of a formulary service across SEL ICS, including GSTT, Kings College Hospital and Lewisham and Greenwich Trust. You will gain significant experience in preparing evaluations, assisting in the development of local medicines treatment pathways and presenting to senior decision makers including consultants and Chief Pharmacists. You will be a key member of both the SEL Joint Formulary Committee and the SEL Integrated Medicines Optimisation Committee. The role will also include a level of clinical commitment, and support for the Medicines Information Department for work relevant to formulary and interface matters.

Main duties of the job
- To support implementation and development of formulary services within the Trust, at KCH and LGT and within the wider healtheconomy of the South East London Integrated Care System (ICS).
- Prepare and distribute the agenda for Joint Formulary Committee (JFC) meetings, including coordinating agenda items with attendeesfrom the 3 acute Trusts as appropriate.
- To perform new medicine evaluations, covering efficacy, safety and financial implications.
- Assist with the maintenance and development of the South East London Joint Medicines Formulary website.
- To answer formulary related enquiries, originating from GSTT and those referred to the formulary team from KCH and LGT.
- To assist with the review and clinical evaluation of unlicensed medicines requests and individual funding requests.
- To assist with the implementation of new medicines within the Trust and SE London ICS, e.g. contributing to updating medicinestreatment pathways.

Guys' and St Thomas' Pharmacy Department is one of the largest in the UK and has a strong focus on individual patient care. Our pharmacists work across multi-disciplinary teams both in the hospital and community setting, to deliver advice, guidance, and support within a culture of medicine safety. The focus is on patient-centred care and this is achieved through providing optimised pharmaceutical care for patients as part of a ward-based and clinical pharmacy service and extending pharmaceutical services beyond traditional boundaries. This includes attendance on consultant-led ward rounds and contributing to multi-disciplinary meetings.
The pharmacy Medicines Information, Governance and Formulary teams are based at Guy's hospital but deliver numerous programs of work and services across all the hospital sites.
