Job summary
An excellent opportunity has arisen for a dynamic, motivated, and innovative pharmacist to join the cardiothoracic transplant pharmacy team at Harefield Hospital in providing transplant pharmacy services from in-patient (including critical care) to out-patients delivering medicines optimisation reviews. As a key member of the multidisciplinary team, you will gain skills in critical care, renal, anti-infectives and transplant; and be responsible for delivering pharmaceutical care to patients who have received a ventricular assist device (VAD) or who are awaiting heart or lung transplantation, and for managing their medicines lifelong following surgery.
Main duties of the job
As a key member of the multidisciplinary team your role will be to:
- Provide and develop clinical pharmacy services for Transplant & VAD patients across inpatients and in medicines optimisation clinics
- Participate in multidisciplinary ward rounds delivering and promoting high quality, cost-effective, patient-focused care including antimicrobial stewardship
- To support the hospital's homecare medicines (including OPAT) service for adult Transplant & VAD patients
- Work closely with the Transplant & VAD teams to promote medicines optimisation: support the development of guidelines, assist in education and training, manage medicines related risk and provide a comprehensive medication service
