Are you passionate about transforming healthcare through prevention, equity, and data-informed care? Join the Population Health Hub (PHH) at Guy's and St Thomas', a pioneering team redefining care for our communities.

The PHH is a multidisciplinary team supported by Guy's and St Thomas' Charity, driving the Trust's ambition to improve population health by 2030. Innovation and learning are at our core.

Why join us? Be at the forefront of improving population health in one of the UK's most diverse communities. Work with a passionate team improving lives and reducing inequalities. Help shape a new care model focused on prevention and equity and contribute to delivering the quintuple aim: better outcomes, equity, experience, staff wellbeing, and value.
