If so, we have an exciting opportunity for you to join the Patch Children's Community Nursing (CCN) team as a Team Leader.
The Children's Community Nursing Service includes the Children's Hospital at home, Community nursing, PATCH and Complex Needs Nursing teams which manage a range of childhood conditions. Children benefit from the expertise of the hospital in the comfort and safety of their own home, school and a variety of other community settings. We have a dynamic team of highly skilled nurses who facilitate early discharge of children from hospital as well as prevent hospital admissions and ED attendance.
The Patch CCN team was developed as part of the Children and Young People's Health Partnership (CYPHP), an innovative quality improvement programme aimed at enhancing the health and well-being of children in Lambeth and Southwark. Our team provides community-based care to children and young people with three ongoing conditions:asthma, eczema, and constipation.
We utilise a population-based approach to healthcare and employ progressive technology to deliver services through community clinics located across multiple sites in Lambeth and Southwark.

Main duties of the job
We are seeking to appoint an enthusiastic and highly motivated Team leader who has a special interest or experience in the management of these long-term conditions.
Applicants will have worked as an autonomous practitioner within the community setting and/or primary care previously. Excellent organisational, time management and communication skills are required to support and lead this forward-thinking team of experienced nurses. The post holder will be a highly visible senior nurse and as such expected to provide specialist knowledge, advice/support, and clinical expertise to the team.
The post holder will work as an autonomous expert practitioner withresponsibility for leading a team of nurses who will provide high qualityspecialist nursing care to children at home and community settings.
In partnership with members of the multidisciplinary nursing team the post holder will develop, implement and evaluate a seamless Children's community nursing service delivery within the Acute Children's Community Nursing Service with a focus on ensuring that patients receive the highest standard of clinical care.
The post holder will be responsible for leading the team and contributing to the ongoing development of clinical practice, research and standards of care within the service, including development of policies, protocols and guidelines and contribute to service developments in collaboration with multidisciplinary colleagues and stakeholders.

To work autonomously, managing a caseload of patients within the specialty whilst working as part of the multidisciplinary team, delivering individualised and personalised direct patient care
Be responsible for assessing and effectively managing conditions within the speciality. To interpret information and take appropriate action and to lead others to do the same.
Act as an advocate for patients, to ensure a patient orientated approach to the delivery of care and to meet standards within the NHS Plan and other relevant guidelines.

Recognise and avoid situations that may be detrimental to the health and wellbeing of the individuals.
Recognise and monitor exacerbations of illness and initiate

To regularly attend and liaise with GPs and acute hospital colleagues to enable timely and safe discharge from hospital and appropriate follow up in the community.
Participate in the promotion of patient centred care, and establish and maintain a supportive relationship with the patient and their family during the period of assessment, treatment and after care
Support and enable patients and carers to make informed decisions
relating to their treatment and management. Provide advice contributing to the wellbeing of the patient and participate in health promotion to improve health and prevent re attendance.
Demonstrate appropriate expert practice and highly specialist advice in the assessment, planning, implementation and evaluation of patient care. This includes history taking, physical examination and the requesting of agreed changes to clinical care.
