We are looking for a dynamic, motivated and enthusiastic experienced diabetes nurse to join the Diabetes service at Guy's & St Thomas which includes our Lambeth and Southwark Community Sites
As a department we are very patient focused supporting innovation and service development to continually improve care for people with diabetes. We are a busy tertiary specialist diabetes centre with over 2000 people with type 1 diabetes and 600 people on Continuous Subcutaneous Insulin Infusion (insulin pumps). We also have a forward-thinking community service across the boroughs of Lambeth and Southwark actively supporting patients with T1 and Type 2 diabetes. Education of patients and other health care colleagues is an essential part of this role
We are looking for a nurse who has experience in diabetes specialist nursing across any of the specialist care pathways. If you enjoy multidisciplinary team working in a busy progressive environment that is continually striving to improve care for the population it serves, this post is for you. We require a nurse who is flexible, dynamic, self-motivated and reliable in addition to being passionate about diabetes and nursing.
The successful candidate will join a relatively large diabetes nursing team that provide specialist care across integrated clinical pathways working across in-patients, specialist out-patients and the community/intermediate care setting.

Main duties of the job
The primary function of the role is to act as highly specialised diabetes nurse working autonomously to assess, treat and prescribe for patients with diabetes and advise non-specialist practitioners on diabetes management plans as required. The specialist diabetes nurses are one integrated team across the community and secondary care diabetes services.
This is a hybrid integrated hospital / community DSN post based within GSTT Diabetes service.
In partnership with members of the multidisciplinary team the post holder will develop, implement and evaluate a seamless specialist service, ensuring that patients receive the highest standard of clinical care. They act as a lead specialist and will demonstrate a high level of expertise within the specialist service, providing specialist advice, education and support to health care professionals and patients. The post holder will actively undertake clinical audits and research within the specialist area.
The post holder will be responsible for contributing to the ongoing development of clinical practice, research and standards of care within the service, including development of policies, protocols and guidelines and contribute to service developments in collaboration with multidisciplinary colleagues. Also ensures the safety and well being of services users, and always working in a manner that promotes dignity and human rights through the adoption of person centered care principles.
