Job summary
*AAC Date Pending Oncology, Ageing Health and Acute and General Medicine Departments have worked together to develop a novel and exciting initiative to improve care for people presenting to non-site-specific cancer services. We are now seeking a new consultant to join our dynamic, enthusiastic and forward-thinking team, to further develop, deliver and embed our innovative models of care.
Specifically, the post-holder will deliver geriatric/general internal medicine expertise in the outpatient Rapid Diagnostic Centre (RDC), on the oncology wards, and support delivery of acute medical care across the Guy's site. The need for such a role has been demonstrated through local work showing that 55% of outpatient RDC attendees are aged over 60 years, with a significant proportion of patients on the oncological wards being older and multimorbid with implications for both acute and longer-term management. The success of the Guys' RDC model has been demonstrated (Dolly et al, BJC 2021) and is being rolled out across the South East London Cancer Alliance. In 2021, the team won the Integrated Healthcare Partnership of the Year Award from the Health Service Journal.

The post holder will work in both the out and in-patient setting to:-
- Develop, implement, and evaluate new pathways of care for older, in- and out-patients with multimorbidity presenting to RDC and on the oncology wards
- Deliver DCC in the STH site UEC pathway
- Provide proactive comprehensive, holistic assessment for patients referred to RDC and on the oncology wards
- Provide geriatric/general medicine expertise on the oncology wards
- Support shared decision making, advance care planning and discharge planning for both in-patients and out patients
- Provide education and training on issues related to general medicine, multimorbidity and geriatric syndromes for the wider multidisciplinary cancer teams

Job Plan
Your job plan is anticipated to contain 10 Programmed Activities per week on average to be carried out on Guys & St Thomas sites. Any on call commitments will be outside of the 10PA post. This is an outline programme and you will be expected to discuss and agree a detailed job plan including your personal and professional objectives with your Clinical Director within 3 months of your start date.
As an organisation, we are committed to developing our services in ways that best suit the needs of our patients. This includes asking certain groups of staff to work more flexibly so that we can offer services to patients in the evenings and at weekends. We regard this flexibility as essential if we are to continue to provide first class patient care in the future. As a result, any offer of employment to a consultant post will be subject to you agreeing to work a new more flexible pattern of working in the future if required and in accordance with the provisions of the new Guys and St Thomas NHS Foundation Trust consultant contract.
Direct Clinical Care Activities
The DCC activities will incorporate ward rounds on the HaemOnc wards, ward based MDTMs, case conferences as required, outpatient clinic three times a week and linkage with the POPS team for MDM.
Other Programmed Activities
Other programmed activities will incorporate undergraduate and postgraduate teaching and training and SPA as listed above
Depending on the interests of the individual they will have an opportunity to lead on the organization and delivery of the foundation year programme in perioperative medicine.
