Job summary
Improving the health of our populations is one of GSTT's five strategic priorities up to 2030. The Population Health Management (PHM) Hub is a newly created team that will deliver a step-change in the Trust's approach to population health, contributing towards the delivery of this priority. Consisting of clinical, analytical and change expertise, it will support clinical services adopt preventative and equitable population health approaches.
By collaborating with clinical services, patients, and key partners across the local health and care system, the PHM Hub will support the design, development, and implementation of insight-led redesigns to our models of care. These initiatives will aim to address the most critical health needs of our local population and reduce disparities in access, experience, and outcomes for our patients.
The Population Health Programme Director will be responsible for non-clinical leadership of the Population Health Management Hub and delivering the Trust's vision for population health and health equity. Working closely with the Consultant in Public Health / Head of the Population Health Management Hub and Deputy CMO (Healthcare Public Health) as part of the Senior Leadership Team for the PHM Hub, to collectively deliver the aims of the population health and equity programme.
Main duties of the job
The main duties of this role will include:
- Provide non-clinical leadership for the Trust's Population Health and Health Equity strategy, ensuring delivery through the Population Health Management hub.
- Ensure the Population Health Management Hub's vision aligns with the Trust and its Partners' priorities.
- Collaborate with key clinicians and clinical services to oversee all Population Health and Health Equity activities and initiatives.
- Develop an effective working model for the Population Health Management Hub, including data and analytics capabilities.
- Ensure robust programme management, strategic delivery, and evaluation of the Population Health and Health Equity agenda.
- Oversee and support the delivery of all programme work streams, enabling key stakeholders to deliver effectively.
- Support clinical service transformation based on population health insights and evidence-based frameworks.
- Enhance the reputation and visibility of the Population Health Management hub and its the clinical service transformation it supports.
- Ensure the long-term sustainability of the Population Health Management Hub beyond the charity grant duration.
