Job summary
We have an exciting opportunity for an experienced Cardiothoracic Nurse to work with the Clinical Nurse Specialist (CNS) team within the Transplant Unit at Harefield Hospital.
The role of the CNS in Transplant Coordination (TXCO) is multi-faceted, encompassing advanced clinical assessment skills and advanced communication/organisational skills. As a CNS in TXCO you will be responsible for providing specialist nursing knowledge and expertise to patients, their families and other healthcare professionals. The post holder will be expected to hold a high degree of personal professional autonomy and as appropriate, make necessary clinical judgements pertaining to the patient group. Service provision is required 24-hours daily, therefore on call is utilised to provide out of hours cover.
Please note a Band 6 may be offered to a suitable candidate who does not have the appropriate experience, training will be given and when competence achieved a Band 7 will be approved.
Main duties of the job
To be a specialised nurse working with a specified group of pre/post cardiothoracic transplant patients, taking responsibility for stages of the journey from referral and follow up to potential transplantation. To provide education/information to cardiothoracic assessment patients and their families regarding transplant assessment and post transplant care/long-term follow-up.To be involved with all aspects of patient reviews (face-to-face; telephone and virtual).Participate in the transplant MDT meetings. Collect relevant audit/research data.Communicate with patients/families; MDT; external health professionals and referring teams.Provide on call cover, as required

- To lead the provision of care in accordance with Trust Policies/Protocols reflecting current trends and developments in nursing and the NHS.
- To provide care, education and support for the designated patient caseload, acting as a specialist resource when liaising with multidisciplinary teams both internally and externally, facilitating the best use of all available services.
- To assess, plan, implement and evaluate programmes of care, for transplant assessment/waiting list patients, utilising evidence-based practice, to improve the quality of care and meet patients needs.
- To facilitate transplant assessment/waiting list out-patient clinics and telephone review clinics.
- To request investigations for designated patient group, as per protocol, interpret and act on results as appropriate.
- To refer, as required, to support services i.e., psychology, dietetics, supportive/palliative care, physiotherapy etc.
- To be aware of the NHSBT Clinical Governance Policy and submit notifications, as appropriate.
- To undertake on-call and/or internal rotation as required, ensuring that there is service cover 24hrs/day, in relation to for the transplant coordinator role.
Communication:
- To work effectively within the trust's transplant team ensuring an individualised, evidence-based service for each patient and their family.
- To act as advocate for transplant assessment/waiting list patients and their families, enabling them to express their needs effectively when dealing with the multidisciplinary team.
- To liaise closely with the Nurse Consultant, Modern Matron, Nurse Specialists regarding the needs of the transplant assessment/waiting-list patients.
- To liaise closely and effectively with NHSBT and the Specialist Nurses for Organ Donation with regards to donor offers/retrieval activity etc.
- To network with other Transplant Coordinators throughout the UK, joining appropriate organisations in order to collaborate, share and benchmark practices and initiatives.
People Management:
- Act as a role model, demonstrating high standards of care and providing clinical leadership in the specialist area.
- To oversee, support and delegate appropriately to junior colleagues.
- To orientate/mentor new transplant coordinators, ensuring competencies are achieved and relevant documentation completed as appropriate.
- To coordinate relevant multidisciplinary personnel when organising transplant activity, both within the trust and at retrieval centre.
- Actively manage own annual leave, in conjunction with colleagues, and in line with Trust and local Policy and Procedure.
Service Development:
- To assist in developing transplant related information for patients, families, and other health care professionals.
- To support and work with the transplant multidisciplinary team in the introduction of clinical change.
- To promote organ transplantation within the Trust and externally as appropriate.
- To be actively involved in the development of transplant related protocols and procedures.
- To keep up to date with advances in treatments/new developments in relation to pre and post transplant research and transplant coordination, and disseminate to team members, as appropriate.

Information Management:
- To ensure accuracy of documentation and to maintain an efficient and accurate record system with relevant information.
- To be responsible for data entry and accurate maintenance of the transplant waiting list and relevant transplant databases.

- To obtain relevant transplant statistics from NHSBT, as required.
- To access and utilise patient information through Trust electronic systems i.e., patient administration system.
