Intermediate Care Southwark
Intermediate Care Southwark offers domiciliary-based rehabilitation and support for clients, who are 18 years and over, to maximise independence. We treat clients with a wide range of functional difficulties, long - term conditions and disabilities.
Intermediate Care Southwark is an integrated partnership across health and social care and uses an enabling approach and goal orientated rehabilitation in order to maximise a client's independent living skills in order to reduce the need for long-term care.
Occupational Therapists will work as part of the wider MDT, comprising of Physiotherapists, Nurses, Pharmacy, Social Workers, Assistant Practitioners, Rehabilitation Support Workers, Senior Rehabilitation Support Workers, Assistant Practitioner OTs and a Specialist Geriatric Consultant.
Main duties of the job
Our service aims to:
- Provide intensive, short term multi-disciplinary rehabilitation promote functional independence.
- Work with Reablement care providers where relevant to optomise functional independence and reducing the care need.
- Support service users to remain living at home as independently as possible, engaging in occupation with support where needed, preventing hospital admission and unnecessary social care placement.
As a Specialist Occupational Therapist in the team, you will have an opportunity to:
- Develop your clinical and reasoning skills in complex moving and handling equipment, minor home adaptations, cognitive assessments
- Develop your skills in complex case management of clients beyond the scope of your traditional Occupational Therapy practice
- Develop your skills in holistic assessment, problem solving and integrated working with other health and social care colleagues, GPs, hospital teams and voluntary sector agencies
- Develop supervisory, leadership and service development skills
- Access appropriate GSTT leadership development training courses to support career development
- Learn, develop and peer-support and network in Occupational therapy clinical meetings

- To delegate rehabilitation and care tasks to RSWs, Senior RSWs and Assistant Practitioner OTs.

- To support the future development of, and participate in, an effective extended service, seven days a week. It is essential the post holder be able to work their hours flexibly and contribute fully to both existing and new rotas over a seven day week, including weekday and weekend on call shifts.
