Job summary
The Nursing and Rehabilitation Support Worker (NRSW) role is multifaceted as it involves the provision of both rehabilitation and/or care interventions, and nursing oriented interventions to clients in their own home, to support Registered Nurses and Therapists. Tasks may include taking clinical observations, simple nursing procedures, teaching prescribed exercise programs, practicing transfers and mobility, assisting and promoting independence in personal care tasks and documenting outcomes of home visits accurately as per trust guidelines.
You will complete home visits independently and alongside other staff members.
You will work as part of a flexible multidisciplinary team (MDT) and undertake delegated tasks from clinicians across all disciplines.
Working pattern will involve regular unsocial hours inclusive of late weekday, and weekend shifts across three patterns of 8.00 - 20.00, 10.00 - 22.00 and 11.00 - 23.00. Your base hours per week are 37.5hours. There are no on-call shifts for this role.
You will communicate effectively with clients, their carers and family members, particularly patients with impairments which may affect their ability to communicate their needs and/or give informed consent to treatment.

The @home Service provides intensive acute intervention for patients in their own home, to prevent hospital admissions and facilitate hospital discharge for acutely unwell patients. This is an exciting time to join the service, with the expansion of the Urgent Community Response function to include health and social care. This is part of NHS England's Long Term Plan to support England's ageing population and those with complex needs.
The @home service are a nursing led team consisting of GP's, Consultant Geriatricians, Nurses, Pharmacists, Occupational Therapists, Physiotherapists, Social Workers, and Nursing and Rehabilitation Support Workers (NRSW). Referrals come from primary and secondary centres, consultants, GPs, health providers, social care services, and the London Ambulance Service.
Patient conditions range from acute medical episodes to more acute on chronic longer-term or complex health or social conditions, including the frail elderly who benefit from short-term acute interventions that our service provides. We work closely with GPs, Hospital Teams, Community Matrons, District Nurses, Community Therapy Teams, and Social and Emergency Care Services.

- To work as an autonomous, lone NRSW implementing holistic pre-formulated care plans to clients in the community, as allocated by Senior Nurses and/or Therapists.
- To hold responsibility for, and manage daily caseload using effective prioritisation and time management skills, and to seek senior support as/when issues arise.
- To work across the boroughs of Lambeth and Southwark when requested as service needs dictate.
- To be professionally and legally responsible for own practice, and accountable for all aspects of treatment delivered to a clients to ensure a high standard of clinical care and a good patient experience is delivered on each contact.
- To deliver basic medical interventions as requested by a clinician including (but not limited to):
- Taking/recording clinical observations
- Monitoring/recording blood glucose levels
- Monitoring medication compliance
- Support medication administration within the scope of practice and trust guidelines/competencies
- Supporting interventions for continence management
- Venepuncture
- Recording of ECG
- Collect specimens (urine and stool)
- Weigh and record routine observations of weight
- Assist with fluid/dietary needs and record intake and output of fluids/solids
- To deliver the prescribed client care plan involving a range of treatments and interventions including (but not limited to):
- Physical and (at times) cognitive rehabilitation to promote functional independence e.g. assisting with, supervising and prompting activities of daily living such as personal care tasks and meal preparation, cleaning and shopping.
- Mobilising clients (indoors/outdoors) and practicing transfers using a range of mobility aids as per care plans. Practicing the use of prescribed adaptive equipment as per care plans.
- Provide therapeutic falls prevention advice/education and implement prescribed exercise treatments to identified at-risk clients.
- Provide clients, family members and carers with informal training in a range of tasks to maximise rehabilitation. This may include manual handling techniques, use of equipment and ADL tasks.
- Assess posture and seating and highlight to Senior Therapists when needs are identified.
- Follow instructions to support patient communication and modified dietary needs
- To use electronic systems to clearly record all contact with clients
