Job summary

Intermediate Care Southwark is an integrated health and social care service. The post holder will be required to work across Southwark Adult Social Care and GSTT integratively to deliver services to residents in the London Borough of Southwark. The aim of this intermediate care service is to provide community based care and rehabilitation to prevent unnecessary hospital admissions, facilitate hospital discharges and to promote independence within the local population.

The team includes: Physiotherapy, Occupational Therapy, Nursing, Pharmacy, Social Work, Rehabilitation Support Workers (RSW), Senior RSWs, Therapy Assistants (TAs), and Business Support. Your role will be to provide specialist nursing for clients allocated to our service. This will primarily but not exclusively include holistic nursing, management of pressure care, education around long-term conditions, continence assessments and support with medication management. The post holder will be based at Queens Road, Peckham.

Nursing staff currently operate on different shift patterns to cover core hours between 9.00-17.00 hours with a requirement to cover on-call shifts from 18:00-20:00 Monday to Sunday. To support the service's commitment to meeting client needs and delivering excellence, it is essential the post holder be able to work their hours flexibly and contribute fully to existing rosters.

Main duties of the job

Professional / Clinical Responsibilities

  • To be competent in using a wide variety of specialised assessment and treatment approaches with adults with a range of conditions, including those with severe physical and cognitive impairment in order to formulate treatment plans, maximise rehabilitation potential and set goals.

Management and Leadership

  • To monitor the quality of nursing/rehabilitation services offered and contribute to the development and review of quality measures and performance indicators.

Communication

  • To be skilled in communicating, using both verbal and non-verbal means, with clients, carers and families who may have a range of physical disabilities or other barriers to understanding.
  • To offer advice and communicate written and verbal information to clients/ carers and agencies including other health professionals regarding a client's management.

Education, Training & Mentorship

  • To educate referrers and clients about the services the team offers.

Audit, Benchmarking & Research

  • To monitor the quality of services offered by the team and when areas of development are identified, to implement an action plan to address these under the direction of the Clinical Manager.

Personal & Professional Development

  • To reflect on practice at all times and to effectively participate in clinical supervision.
  • To maintain Nursing and Midwifery Council (NMC) registration and to work within NMC code of conduct.
