The Departments of Oncology, General Medicine and Ageing and Health have worked together to develop a novel and exciting initiative to improve care for people presenting to cancer services.
We are looking to recruit an enthusiastic, motivated experienced GP with a special interest as a diagnostician to join the Rapid Diagnostic Clinic team in South East London. The role will be based at GSTT with the further option to work in other South East London locations in line with service expansion. The SEL RDC is a HSJ award winning team tasked with diagnosing cancer and other serious conditions. The RDC's are a key part of the NHSE ambition to diagnose all Cancer by stage 1 or 2 by 2028. This substantive full time 10 PA post offers an exciting opportunity to join these innovative teams at GSTT. Applicants who would prefer part time post are welcome to apply. The post has been established to broaden the scope of the RDC in South East London and continue in developing close working relationships between primary and secondary care to better cater for the needs of our patients and the wider RDC programme in South East London.
Main duties of the job
Candidates are not expected to have received subspecialty training in oncology but should demonstrate enthusiasm and track record in innovative service delivery and evaluation, clinical skills in evaluating complex medical patients, education and training and quality improvement programmes. The successful candidate will be expected to be intellectually flexible and able to look beyond existing structures, ways of working and traditional care boundaries in order to produce more effective and innovative service delivery. They will be expected to take an active role in research and development and to participate in the teaching programs of the department. The post-holder will need to adapt quickly to new situations and provide high level clinical care to complex in and out-patients.
The post holder will work in both the out and in-patient setting to:-
- Develop, implement, and evaluate new pathways of care for in- and out-patients with multimorbidity, polypharmacy and early diagnosis of cancer of serious benign conditions
- Provide proactive comprehensive, holistic assessment for patients referred to RDC
- Provide education and training on issues related to multimorbidity and medical conditions for the wider multidisciplinary cancer teams

Job responsibilities
The post-holder will be accountable to Dr Hartmut Kristeleit, Clinical Director for Oncology but will have strong links with James Fleet, Clinical Director of Acute & General Medicine. The post holder will report to Dr Saoirse Dolly, Clinical lead for RDC.
Specifically, the post-holder will deliver general medical expertise in the outpatient Rapid Diagnostic Centre (RDC) and support delivery of acute medical care across the Guys site. The need for such a role has been demonstrated through local work showing that 55% of outpatient RDC attendees are aged over 60 years, with a significant proportion of patients on the oncological wards being older and multimorbid with implications for both acute and longer-term management. The success of the Guys RDC model has been demonstrated (Dolly et al, BJC 2021) and is being rolled out across the South East London Cancer Alliance. The team recently won the Integrated Healthcare Partnership Award of the Year from the Health Service Journal.
The key roles and responsibilities of the role will be:
- To undertake face to face and virtual RDC clinics
- Oversee Advanced Nurse Practitioner telephone assessment
- Participating in RDC MDMs
- Service development between RDC and primary care
- Involvement in SEL RDC education work inc. ANP and GP education
- Option to undertake audit/ RDC research
Provide High Quality Care to Patients
- The post holder must be medically qualified and maintain GMC specialist registration and hold a licence to practice in relevant specialties
- To develop and maintain the competencies required to carry out the duties required of the post.
- To ensure prompt attendance at agreed direct clinical care Programmed Activities.
- To ensure patients are involved in decisions about their care and to respond to their views.
Research, Teaching and Training
- Where possible to collaborate with academic and clinical colleagues to enhance the Trusts translational research portfolio, at all times meeting the full requirements of Research Governance.
- To provide high quality teaching to medical undergraduates and members of other health care professions as required by the Clinical Director.
- To act as educational supervisor and appraiser as delegated by the Clinical Director to ensure external accreditation of training post.
Performance Management
- To work with medical, nursing and managerial colleagues to ensure high performance in the following areas:
- Clinical efficiency e.g. LOS reductions, reducing cancelled operations and DNA rates.
- Quality of outcomes e.g. infection control targets, reducing re-admission rates.
- Financial management e.g. identification, implementation and achievement of cost improvement programmes and participating in efforts to ensure services are provided cost effectively e.g. managing locum agency spend, monitoring and managing the drug budget to target, ensuring accuracy of clinical data for the team.
- Operational efficiency e.g. day-case rates, waiting list activity and demand management.
Medical Staff Management
- To work with colleagues to ensure junior doctors hours are compliant in line with EWTD and New Deal.
- To ensure that adequate systems and procedures are in place to control and monitor leave for junior medical staff and to ensure that there is appropriate cover within the clinical areas, including on-call commitments
- To participate in the recruitment of junior medical staff as and when required.
- To participate in team objective setting as part of the annual job planning cycle.
- To be responsible for the annual appraisal of all doctors in training, Trust doctors, Clinical Fellows and non-consultant grades as delegated by the Clinical Director/General Manager.
Governance
- To review clinical outcomes in designated area using external benchmarking data where appropriate, to identify and advise variances to the Clinical Director.
- Participate in clinical audit, incident reporting and analysis and to ensure resulting actions are implemented.
- To work closely with the Directorate, Patient and Public Involvement panels in relation to clinical and services developments as delegated by the Clinical Director.
- Participate in ensuring NICE requirements are reviewed and implemented and monitored in the speciality areas.
- To ensure clinical guidelines and protocols are adhered to by junior medical staff and updated on a regular basis.
- To keep fully informed about best practice in the speciality areas and ensure implications for practice changes are discussed with the Clinical Director.
