Band 6 Deputy Clinical Nurse Specialist - Lane Fox Home Ventilation Service.
An exciting opportunity has arisen within the Lane Fox Home Ventilation Service. We are looking for a highly motivated nurse wishing to develop their already existing knowledge and skills in Long Term Ventilation and independent decision-making skills.
The Lane Fox Unit at Guy's and St Thomas' NHS Foundation Trust is a tertiary referral centre and the Home Ventilation Service supports patients requiring Long Term Ventilation outside of the hospital environment across the South East of England. There are currently over 200 patients seen by the team who require either Non-Invasive Ventilation or Tracheostomy Ventilation.
Working as part a team, you will be expected to manage your own caseload of patients requiring Non Invasive Ventilation in the community, as well as support tracheostomy tube changes at hospital-based clinics and in the community. There will be opportunities to develop the role and the successful applicant will be able to participate in ward rounds at nursing homes within the Long Term Tracheostomy Ventilation Care Facility Network, as well as referring hospitals across the region.
The successful candidate must hold a full UK driving licence and be able to drive across the South of England according to service need.
Main duties of the job
- Work independently to manage a caseload of Level 2 patients in the community who require Non Invasive Ventilation, whilst supported by Clinical Nurse Specialists already in the team.
- Participate in routine tracheostomy tube changes at hospital-based clinics and in the community alongside the CNS team.
- Participate in ward rounds at nursing homes within the Long Term Tracheostomy Ventilation Care Facility Network.
- Triage referrals to the team and review patients in their home environment in a timely manner.
- Facilitate training sessions for care staff looking after patients requiring NIV.
- Deputise for the CNS team in their absence.
- Liaise with the inpatient service to maintain links between the inpatient and outpatient settings.
- Represent the Lane Fox Unit, PACCS Directorate and Guy's and St Thomas' at local, regional and international conferences relevant to the specialty.
- Drive across the South of England to review patients in their own home environment, in line with Lone Worker Policy.
- Document visits and perform outcomes on the EPIC system.
