Our Integrated Health and Social Care service works with adults aged 18 years and older living in the London Borough of Southwark. The people we support have a range of diagnoses and clinical presentations, including falls, orthopaedic, respiratory, frailty and neurological conditions. You will deliver high quality interventions as part of an integrated multidisciplinary team, including Occupational Therapists, Nurses, Pharmacists, Social Workers, Rehabilitation Support Workers and a Consultant Geriatrician.
Our service aims to:
- Prevent unnecessary hospital admissions.
- Facilitate the safe discharge of people home from hospital.
- Provide intensive, short term multi-disciplinary rehabilitation and care support to promote functional independence.
Main duties of the job
As a Specialist Physiotherapist in the team, you will have an opportunity to:
- Develop your clinical skills in complex case management of clients beyond the scope of your traditional physiotherapy practice
- Develop your skills in holistic assessment and problem-solving, and integrated working with other health and social care colleagues, GPs, hospital teams and voluntary sector agencies
- Develop supervisory, leadership and service development skills
- Access appropriate GSTT leadership development training courses to support career development
- Learn, develop and peer-support and network in physiotherapy clinical meetings
- Access to a mentor

- Degree in Physiotherapy or recognised equivalent
- HCPC registered Physiotherapist
Desirable
- Evidence of post registration education in specialist area
- Member of Chartered Society of Physiotherapy

- Significant broad based post registration experience at band 5 or above
- Experience of working in a rehabilitation team environment
- Able to manage staff conflict and difficult situations and to challenge and support others appropriately and assertively
- Experience in carrying out complex assessments and health screening

- Experience of clinical audit, benchmarking and evaluation of services
- Experience of direct supervision of more junior staff, and experience of educating and assessing students or junior staff
