Job summary
The Integrated Respiratory Team (IRT) provides care for patients residing in Lambeth or Southwark or accessing GSTT through the acute and outpatient services, who have a diagnosis of COPD or asthma, and those who require home oxygen or pulmonary rehabilitation. A new and exciting opportunity for a B7 Clinical Nurse Specialist has arisen to support the expansion of the respiratory virtual wards across Lambeth and Southwark to provide admission avoidance and early supported discharge pathways for patients being treated for an exacerbation of COPD. In this role, you will be responsible for assessing and identifying patients for remote health monitoring and gain consent. The successful candidate will be required to work within the acute hospital setting and visit patients in the community, you will be responsible for providing virtual ward support within house as well as working with our partner organisation DOCCLA to provide digitally enabled remote monitoring. Excellent communication skills and IT skills will be required to ensure that patients are added to the IRT Virtual Wards area on the GSTT Electronic Health Record (EPIC) and the post holder will need to have experience with COPD management plans.
The post holder will be required to work 4 days (8-6pm) a week across the 7 day service.

Main duties of the job
As part of the IRT team the successful candidate will work closely with other members of the team including Clinical Nurse specialists, Physiotherapists, pharmacists and doctors as well as our partner agency DOCCLA. You will be supported by an Advanced Nurse Practitioner and Matron and will work autonomously as part of the multidisciplinary team, delivering individualised and personalised direct patient care- Review patients in A&E, on the ward, at home and on the virtual ward with exacerbations of COPD and provide appropriate acute treatment and long term management.- Provide patients with an exacerbation of COPD or Asthma with education about their condition and how to self-manage plans.- Complete Asthma and COPD bundles with patients.- Provide breathlessness management education- Assess and review patients for long term oxygen therapy, palliative oxygen therapy or ambulatory oxygen.- Provide advice to patients, carers or to health professionals via our Integrated Respiratory Team advice line.- Provide induction training to rotational physiotherapists and new nursing staff joining the team.- Contribute towards in-service teaching, as well as teaching to other teams within the organisation.- Provide leadership, clinical supervision and take responsibility complete supervision/PDR sessions.- Contribute towards projects and research to help develop the services we deliver.- Contribute to the collection of data for national audits
