Job summary
We are looking for registered Health Visitors to join our dynamic Early Intervention Health Visiting Service to deliver an enhanced package of care to children and their families living in Lambeth and Southwark.
This is an exciting opportunity to develop, shape and embed the Bright Beginnings pathway, tailored to support children with multiple/complex needs. You will use your experience and skills to build therapeutic relationships with families and work closely with our partners in Lambeth and/or Southwark to deliver packages of care to a defined caseload of children.
As an Early Intervention Health Visitor, you will use your specialist community public health nursing skills to provide a high level of care and utilise research based assessment and support tools with families.
Working within Lambeth and/or Southwark, you will be based in one of our main offices and visiting children and their families at home and community clinics in an agile way.

Main duties of the job
The post holder will be the allocated Early Intervention Health Visitor for a defined caseload of children.
You will assess complex health needs and provide packages of care/interventions, health promotion and advise on the prevention of ill health for children and their families on the allocated caseload.
The post holder will work with families in their homes as well as at community venues, utilising a case management approach and structured parenting interventions to improve outcomes for children.
The Early Intervention Health Visitor is required to work closely with families and multi-disciplinary partners in addressing a range of complex needs. They will contribute to the safeguarding agenda in line with Pan London Safeguarding Children's Partnership Board and "Working Together to Safeguarding Children" (2018)
