Administrator - @Home
An exciting opportunity has arisen for an enthusiastic and motivated individual to join the @Home team as administrator.
The @Home team, who are currently an accelerator site for Urgent Community Response across South East London, is a Hospital at Home service covering both Lambeth and Southwark. The service provides intensive multidisciplinary acute clinical care at home that would otherwise be carried out in the hospital.
Main duties of the job
This role will involve duties such as managing referrals received via the Unified Point of Access, providing clinical support to our large multi-disciplinary team and other administrative tasks such as managing an inbox and taking patient or clinical queries over the phone.
This role will involve shifts between 08:00 and 20:00 and weekend working. One weekend to be worked in four weeks (once a month) on average.
The post is based across 2 sites, Elmcourt Clinic (Tulse Hill, SE27) & Walworth Road Clinic (Walworth, SE17).
About us
The service operates 7 days per week, from 08.00hrs to 23.00hrs. Patients using the service range from those requiring a single episode of care e.g. postoperative to IV therapy for rehydration, antibiotic therapy; through to patients living with complex and long term conditions, including the frail elderly, who may require periodic short term intervention to maintain the best possible quality of life at home. The service accepts referrals from GPs, London Ambulance Service (LAS), Guy's and St Thomas' Hospitals, King's College Hospital, St George's Hospital as well as other community services.
@Home encompasses two main streams of work:
- Same day or next day response to support early discharge from hospitals (Guy's & St Thomas', King's College), integrated with secondary care.
- Hospital admission avoidance, with 2 hour or same day response, working in conjunction with London Ambulance Service (LAS), GPs, Community Matrons, District Nurses, Social Care and the other community therapy teams.

- They will have good knowledge of IT systems and be competent in Microsoft Office packages. Ideally, they will be familiar with electronic patient record systems such as Carenotes and PiMS.
