An exciting opportunity has arisen to join Evelina London Children's Hospital in our dedicated, friendly and progressive Children's Community Nursing Service. Are you an experienced, enthusiastic, motivated band 5 or already in a Band 6 role? Experience working in the community is preferred but not essential - we are interested in the skill set you have that you can bring to our team.
We are a passionate team of highly skilled nurses who facilitate early discharge of children from hospital as well as prevent hospital admissions and ED attendance. We are seeking two dynamic and motivated Senior Nurses to join our friendly well-established CCN team that delivers family-centered nursing care to children with acute, complex, and long-term conditions in the London boroughs of Lambeth and Southwark. Based from Sunshine House, Peckham, you will be a highly motivated independent practitioner with sound evidence-based skills. The post holder will manage a varied caseload, act as a resource for an identified area of clinical special interest, and contribute to the ongoing development of the CCN team.

Main duties of the job
The Children's Community Nursing Service includes the Children's at Home, Community nursing team, Continuing Care Nurses and Complex Needs School Nurses who manage a range of childhood conditions. Children benefit from specialist hospital care closer to home.
The post holder will provide clinical supervision to other staff, students and may undertake research, lead clinical audits in own specialist area. Ensuring the safety and well being of services users, and always working in a manner that promotes dignity and human rights through the adoption of person centered care principles.

You will need to be an autonomous practitioner, within a team, that provides high quality specialist nursing care to children in their homes and primary care settings. You will also undertake training in mainstream schools and nurseries.
There will be the opportunity to rotate within our wider CCNS team as the service is constantly evolving. This is an exciting opportunity to help shape and build a comprehensive childrens nursing service for the local children, young people, and families of Lambeth and Southwark.
Clinical and Patient CareIndependently manage a group of patients within the specialty whilstworking as part of the multidisciplinary team, delivering specialisedindividualised and personalised direct patient care.
Be responsible for assessing and recognising emergencies in thespeciality. To interpret information and take appropriate action.
Demonstrate appropriate practice and specialist advice in the assessment,planning, implementation and evaluation of patient care. This includeshistory taking, physical examination and the requesting of agreeddiagnostic procedures/clinical investigations.
Responsible for the correct administration of prescribed medicationincluding transfusion of blood and blood products, to ensure safe custodyof medications in the home setting.
Be responsible for assessing and recognising emergencies in thespeciality. To interpret information and take appropriate action.
Act as an advocate for patients, to ensure a patient orientated approach to the delivery of care and to meet standards within the NHS Plan and other relevant guidelines.
Ensure that patients receive high quality clinical care and a good patientexperience, having regard for their customs, religious beliefs anddoctrines.
Recognise and avoid situations that may be detrimental to the health andwellbeing of the individuals.
Recognise and monitor acute exacerbations of illness and initiateappropriate nursing interventions.
To carry our specialist procedures in the community settings egadministration of intravenous antibiotics, care of central venous accessdevices and changing of naso gastric tubes.
Is wholly accountable for his / her practice in line with the NMC code ofprofessional conduct and takes every reasonable opportunity to sustainand improve his / her knowledge and professional competence.
