Job summary

Perioperative medicine for older people undergoing surgery (POPS) is an award-winning service and the first of its kind in the UK. It started in 2003 and has rapidly become an essential surgical support service. It is recognised locally and nationally as being of high quality, innovative and clinically effective. POPS looks after patients aged 65 years and above and aims to improve their medical health before and after surgery by assessing them before surgery, following their care while in hospital and supporting consultants and ward staff.

The post holder will have contact with the patient at all stages of the patient journey. They will be supported by a consultant physician for clinical supervision.

They will gain experience in the use of comprehensive geriatric assessment (CGA) in the surgical patient, learning how to apply this in the elective and surgical patient group.

They will develop skills in the recognition of increased surgical risk and common post-operative complications. The post holder will gain skills in how to manage these issues and its impact on long term health.

Main duties of the job

Duties and responsibilities

- Weekly outpatient clinics

- Attending POPS multidisciplinary team meetings

- Inpatient reviews on surgical wards

- Presence at ward MDT meetings with a focus on discharge planning

In addition to regular visits to the surgical ward the CNS will

  • rapidly respond to acute issues and management of post-operative problems on the surgical ward
  • The post holder will monitor patient referrals to POPS including screening of older patients
  • liaise with surgical pre-assessment teams and waiting list managers to coordinate surgical admissions
  • liaise as appropriate with social services, intermediate care teams, primary care and voluntary agencies in order to implement the required services for 'POPS' patients
  • ensure high quality communication with the patient, relatives and carers
  • support patients and carers through their surgical journey
  • attend and contribute to LOS/Cluster meetings in Surgery
